Over-exertion (Not Elsewhere Classified)

Employee was climbing down some steps, and when EE stepped off EE twisted EE's left ankle. EE went to a prompt care facility, then was sent for x-rays. EE broke EE's left ankle.

Employee was exiting a skid loader, when they stepped on a rock and hurt their right ankle. The pain and swelling increased as the day went on, so they were sent to a prompt care facility that afternoon. Employee was examined and sent to a hospital the next day for further evaluation. They had a slight fracture in their ankle. Employee returned to work the next Monday.

Struck against stationary object

Employee was operating a Cat 988 loader, attempting to cut the pit floor down. The bucket was at an angle and it dug in, stopping the loader abruptly. The employee went forward and hit head on the windshield. EE complained of a headache , so EE was sent to the hospital to get checked out. EE was diagnosed with a slight concussion.

Employee was feeding a gravel plant with a Cat loader. There is a heater fan in the cab of the loader. The employee said the fan was blowing cold air on EE. EE tried to stuff a rag into the fan. Apparently EE's hand went into the fan, and EE cut the second and third digits of EE's right hand.

Struck by flying object

Employee said was around a grinding instrument and dust, and it felt like EE got something in left eye. EE went to the doctor. The doctor did not find any foreign object in EE's eye, but thought there might be a slight abrasion. Doctor prescribed eye drops. The employee went back 2 days later for a follow-up. Everything was fine.

Employee was working on a radiator shroud for a dragline. When handling the shroud, he scraped his left ring finger. He washed it and went back to work. He ended up going to the emergency room on July 4. They determined he had an infection in his finger. He did not report the incident to his employer until July 6.

Fall to the walkway or working surface

Customer (Truck Driver) stopped at Scale Office to tell them what she wanted. When returning to her truck she slipped and fell on snow & ice on the road surface where she had parked. Weather conditions on 11/23/05 were snow & blowing, Customer slipped on flat ground approaching her truck breaking her hip.

EE WAS CHANGING DRAG CABLE ON CRANE, USING A PRYBAR. THE PRY BAR SLIPPED, & HE HIT HIS LEFT HAND ON THE EDGE OF THE DRUM.
