EE sat down on the operator seat of a 4 wheel personnel carrier. When EE raised a leg to swing it into the 4 wheeler EE felt a sharp pain across lower back.
During workday EE used a sledge hammer, carried boxes of blasting caps and other tasks. When shift was over they were experiencing pain in neck and shoulders. EE said it felt like a pinched nerve. Pin was frozen to correct temperature and both were holding each side of the 180 lb. pin when customers job mechanic stumbled. EE took the whole weight of the pin to keep it from falling on self. Felt burning sensation in stomach.
Employee was operating a 785D rock truck on Valley Fill #9. Employee stated they were cleaning the mirror on the offside of the truck and when they raised up they struck their left shoulder on the handrail on the truck.
EE was working on a 785 truck at Grape Vine South Surface removing the main pump with another EE and a field person from JP Tech. Pump was almost out, EE moved to the side and was waiting for pump to come out to the ground. When it did, it struck the pry bar EE had been using. When the pump fell to the ground, it hit the pry bar and the bar slammed into EE's lower leg.
Employee was bending over a pull slack out of conveyor belt when he felt a sharp in his lower back.
Employee had taken his lunch break and was climbing up the ladder on a 777 rock truck when he felt sharp pain in his right shoulder and lower back.
EE was removing cutting edge from a D-10 dozer. He was removing what he thought was the 2nd to the last bolt holding the cutting edge on however, the remaining bolt had broken off and when ee removed the bolt the cutting edge flew loose due to dirt behind the cutting edge and the cutting edge came forward and struck him on the forehead. He sustained a laceration requiring stitches
Employee and 2 other employee's were moving a water pump from a muddy area that had already been pumped down to another location. The pump weighed approximately 75 pounds but muddy conditions made moving the pump more difficult. Later that day employee began experiencing pain in his lower back and down his right leg. Employee became a lost time on 5/5/2014.
Changing bits on a miner machine and right finger got struck on a bit causing a laceration
EE was preparing to weld a DM45 drill jack pad on the bottom of the jack. He was moving the pad closer to the drill when he felt pain in his lower back. EE continued to work until 4/17/13 when he sought medical attention for pain in his back and leg. The doctor took him off work until 4/24/13.
Employee was unloading a tool box from a 3 wheeler. The tool box slipped and when he tried to catch it, he twisted his back. Employee's first day away from work was 8/21/2012.
While changing a tire on a 785 rock truck finger were mashed between the lock ring and the bead seat of the wheel.

EE's left middle finger was caught between the blocking and four wheeler. He was wearing gloves but still sustained a laceration requiring stithces.

EE was repairing cat pads on a continuous miner using a wire cable, the cable was frayed and it caught in his right ring finger. He was taken to St. Mary's Hospital in Huntington, West Virginia. The cable was removed and EE was placed on restricted duty.

Employee was removing a fuel injuector line while lying on his back and stretching & twisting to reach the clamps on the line. He felt a popping in his back. Employee did not seek treatment until 11/12/2008.
Employee was working to insert auger screw back into barrel using a backhoe to insert screw when the key steel that lines the screw with the planetary gear started to come out. He used his finger to push the key steel back in and it got pinned between the collar of the screw and the top of the mast requiring a surgical procedure.
EE was shoveling loose coal around the 3A Belthead when he felt pain in his right shoulder and a knot erupted on the shoulder. EE did not miss work,nor did he produce any excuse from a doctor for light duty. Compensation has not been billed for any injury. He was not performing his regular duty when this incident occurred.
