Accident type, without injuries

Based on our initial investigation, it appears a hydraulic hose ruptured on the dozer and sprayed on the turbo. The operator noticed flames coming out of the engine compartment and exited the dozer.

Climbing onto a D9R tractor using 3 points of contact when ee pulled up ee felt a sharp pain in the stomach area.

An anonymous caller informed MSHA there was a fire at stockpile #4. There were no flames in the stockpile but steam and smoke were present from the dozer moving the coal to cool it down.

An anonymous caller informed MSHA there was a fire at stockpile #4. The flame was approximately 6 inches high and 2' by 2' in diameter. The pre-shifter spotted the flames and tamped them out with an excavator. The flames were out within 15 to 20 minutes.

Employee was operating a D9R Dozer on the coal stockpile when without warning the coal broke loose under the left track of the dozer. The dozer pivoted downward into a void and was encased in coal. The operator was uninjured but he was entrapped longer than 30 minutes.
