Employee was attempting to pump the pit using a 3 inch water pump. Employee turned pump on and believed it was operating correctly. After returning to the pump 30-40 minutes later employee realized the pump was not sucking. Employee shut the pump off & released the hose causing hot water to be sprayed on the employee. Employee received 2nd degree burns on arms & hands.

Showing all 10After changing tire, employee lifted the tire into the back of the truck and in doing so injured intestine. Employee finished shift, but advised that employee thought maybe had pulled a muscle in stomach. After the shift, employee went to the hospital. They were pumping stomach to avoid surgery, but doctor advised that employee will need surgery on small tear on intestine.
EE was entering operation shack of dredge when a gust of wind blew operator into shack. EE caught self on the door frame with EE's left hand when the door shut pinching the index finger on EE's left hand. After EE's shift, EE went to Prowers Medical Center to get EE's finger cleaned up. The tip to EE's finger was amputated at the time of the incident.
Employee climbed surge bin to adjust gate flow. During process of adjusting the nut, employee fell approximately 8'. Employee was not wearing fall protection. A ladder and man lift were also available for use. Employee has been trained on fall protection.
Employee was trying to change Skid-steer bucket attachment. The pins were not releasing due to buildup of frozen mud/ice. After getting bucket free, the bucket fell about 2' and landed on employee's foot behind the steel-toe protector. The result is a fracture in metatarsal bone.
Employee was pulling on a jammed gate. When the gate gave way, the gate pinned his right thumb between a large bolt.
He was shoveling rock out from the rock crusher and either strained his back shoveling or reaching for a shovel.
