Employee was cribbing up the bumper of miner. The miner sit down on the end of the crib block, causing the other end of the crib to strike employee in the chin, causing a laceration.
The Employee stated: that EE was bending a roof bolt on a flat surface, the roof bolt twisted around while EE was bending it and EE felt a strain on the right side of EE's lower back.
Employee lifted a 7' timber twisted around felt a sharp pain in the middle lower back, resulting in a strain. Employee was at x-cut 25 on the Main North Supply Road.
Employee stated; was unloading 20ft sections of 4 inch fresh water line on #2 unit. Employee was working alone when they stated that they felt pain in their lower back on the left side.
3 individuals were in the process of replacing the lid on the power center. The injured person was on one side of the lid and the other two were on the opposite side of the lid. As they were placing the lid in place two of them accidentally dropped their end. The injured employees hand was underneath allowing the lid to land on left index finger.
Employee Stated: EE was building a brattice. EE turned around in a twisting motion and bent over, picked up a block and raised up turned around and felt a pain in lower back on the right side.
EE states got off ram car to remove flagging and to make sure it was safe to travel through cross cut and while getting back in car felt pain in lower back.

Employee stated he was closing airlock door at the mouth of # 1 unit and timber worked loose and door fell striking him on his left hand on ring finger cutting at first joint of finger. Took to Ohio co. ER and received four stitches
Employee was on #3 unit entry #7 working on the pre-dump of double boom fletcher roof bolter. Employee told the operator to try the pr-dump. As employee was getting up off the mine floor he became off balance and grabbed the pr-dump with his left hand and the pre-dump closed on it resulting stitches to his middle finger and index finger.
Employee was hanging continuous miner cable on #2 unit in #6 entry and strained his back.
Employee stated: was moving power on #2 unit and was in #6 entry moving power cables. There was mud and water in this entry and was dragging cables thru this area resulting in straining right side of lower back.
Employee stated: Installing top Roller belt framing on the #4 unit entry #4 when thumb on the left hand became caught between the framing and the rope resulting a fracture to the end of the left thumb.
Employee stated: was moving belt air lock on #3 unit entry #4 while bending over to pick up timber felt lower back knot up and could not straighten up.
Employee was in the plant thickner tank helping clean out mud when he pulled on a pump hose to reposition it. The hose was stuck in mud and was hard to move. He started having lower back pain after he went home. Employee missed his first scheduled day of work on 7/6/2015.
Employee was in the shop on the surface bending a piece of metal in the bending press. While bending a piece of metal, the metal broke and struck right index finger resulting in a laceration in the bend of the finger. Employee received five stitches.
Employee was on #4 unit entry #5 working on the Feeder. He put pin in the cat assembly and removed come along off the cat assembly. They lowered the cat assembly with the hydraulics of the feeder and as the assembly was coming down the cat caught the tip of the hook and slip off striking him in the mouth and busted one tooth.
EE was on #3 unit supply road loading old pallets onto a trailer. EE picked pallet up and twisted to throw pallet onto the trailer which resulted in a strain to the lower left side of his back.
Injured employee stated he was loading supplies onto the roof bolter and pulled a muscle in his lower back.
Employee was shoveling belt at the 1C Header, when he felt a pain in his back. He did not report this incident until 03/11/2015 and brought in a doctors excuse missing today (03/30/2015)
#1 unit #7 entry in the last open cross cut. EE was loading glue on back of the roof bolter. While turning back around to put glue on the bolter, right side of head struck damaged pie pan on roof bolt. Piece of pie pan went under the side of hard hat causing laceration three inches above right ear resulting in two staples.
Employee was attempting to install a piece of expanded metal over a chute at the breaker to create a walkway, the metal slipped. As he attempted to catch it, he put all of the weight on his right foot which caused the injury.
Was bolting on #3 unit entry #9 at the face. Made roof bolt up and walked to front of the bolter and turned to get bolt. When turning back around right shoulder struck pie pan resulting in laceration. Received 9 stitches.
While trying to shove a hose in drain line on spirals that was stopped up he twisted his back. Employee did not miss a scheduled work until 2/9/2015.
Injured employee was holding up a 3/4" plywood door while hinges were being installed when his right knee gave out.

EE states: was throwing concrete block and strained left shoulder. This accident occurred 2/24/2014 as reported only. I was just notified yesterday that he had surgery thru Human Resources.
