Driller was in the process of drilling boreholes, dust hose collection was not operating properly. Upon inspection of the dust collection hose, driller saw an obstruction. EE removed the hose to clear debris from the hose and upon re-attaching the dust collector hose, EE's right hand came in contact with the metal fastener on the hose, cutting EE's right hand.

Employee was putting new gears in a shaker. The gears slipped off of the shaft and employee's right thumb got caught between the gears and the shaft when they fell. Employee's right thumb was fractured and cut.

The EE was working in the shingle shack where they make slate roofing shingles. EE was operating the trimmer saw for cutting the slate shingles. EE was cleaning the cutting table from debris from slate clippings. When EE wiped the surface EE stuck EE's hand in to far resulting in cutting the tip of EE's middle finger.

Employee was attempting to set a drill from vertical drilling to horizontal. The drill mast was laid back toward the power section at about a 45 degree angle. A pin was placed in the hydraulic positioning cylinder to hold the mast in place. The locking pin fell out allowing the mast to swing downward toward the ground. The mast swung down striking the employee in the lower leg.

EE had pulled a piece of rock to the edge of the mechanical conveyor and then turned back to grab tools without communicating with the saw operator. The saw operator activated the conveyor and the rock fell off the edge onto the back of ee knee. This accident occurred in our roofing mill where employees work on the production line making slate roofing shingles.

Injured EE was connecting lifting strap from Bearing to Crane Hook. Bearing was propped to provide access to opening for strap connection. Bearing shifted from raised position and EE was suddenly supporting most of the weight.

Miner was positioning screening cloth on top screening deck when his pinky finger got cut between the cloth and screening structure. Miner was not wearing gloves when the accident occurred. Miners finger required stitches which makes this accident reportable
