Employee was holding a bolt with a pair of locking pliers. Another employee was using a reciprocating saw to cut the bolt. When the bolt was but through, the saw sung up and cut the employee's finger.

Employee was cutting an information from a hydraulic hose with carpet knife, knife slipped and cut thumb on left hand.

Miner was walking down the rock ramp that had been dug out at the surge pile to reach the feeder. The miner was part of a crew working on the surge pile feeder. It had been raining all morning and the rock was wet. The employee slipped and cut a finger on the rock as they caught themselves.

Driller had placed a cardboard blast hole tube into a completed hole, while cutting to proper height of tube above the ground. EE was pulling EE's cut off tool toward self, tool slipped sticking in EE's right lower leg.

Over-exertion in lifting objects

Injured EE was connecting lifting strap from Bearing to Crane Hook. Bearing was propped to provide access to opening for strap connection. Bearing shifted from raised position and EE was suddenly supporting most of the weight.

EE was driving a Terex 3066 C Articulated Rear Dump Truck down a steep gravel haul road into a turn. The equipment's rear tandem tires began to ""buck jump"". EE applied brakes. Truck slid sideways and turned over on right side. EE caught arm in steering wheel and broke right humerus.

Employee was using a pole to clean out the pug-mill hopper. When the employee thrust the pole into the material, he hit the outside edge of his right hand on the corner of a drive belt guard. This caused a cut in the hand and 7 stitches were required.

Employee was chipping a recent weld when some hot slag was chipped onto his left elbow. The employee waited two weeks to seek medical attention when the elbow became red and swollen. He did not indicate injury was definitely he related until 10/09/06 ( the return to work date).

The employee was dipping the sediment pond using the track hoe. Some of the mud fell out of the bucket, hit the handrail on the track hoe, ricocheted through the open window and hit the employee in the corner of his left eye. The employee was wearing safety glasses.

HE WAS IN THE CENTER OF PLATFORM WHEN HE CUT THE PLATE TO 27""X96"", THE PLATE FELL 11'3"" TO THE GROUND WITH THE EE LANDING ON TOP.
