Employee was welding and grinding at the plant and got a small piece of metal lodged in his eye. The doctor had to removed the piece and applied ointment to eye. Employee missed one day of work.

Fall down stairs

The miner was walking down a 5 foot staircase from the MCC to the ground, when his foot caught the top stair slightly causing him to lose his balance and fall to the bottom of the staircase. The only injury acquired was a 1 inch cut to his upper lip from contact with the ground.

Employee was cutting holes in a steel plate under the D8 Dozer, when a gust of wind blew a piece of molten slag into his ear, burning his ear.

Fall from machine

Employee was descending the ladder from his haul truck when he stepped onto the ground he stepped on a rock about the size of a salt shaker. The rock rolled causing the employee to twist his ankle and fall. As he fell he tried to catch himself with his left hand, causing a sprained wrist and elbow contusion.

Employee was replacing the lid onto the impact crusher, as he turned the lid into place it pinched his finger causing a laceration.

THE INJURY OCCURRED TO A CONTRACTOR EE WHO HAD BEEN HOLDING A STEEL PIN STUB WHILE ANOTHER EE DROVE THE STUB INTO THE GROUND WITH A SLEDGE HAMMER. THE STUB HOLDER THOUGHT THE STUB WAS IN THE GROUND WELL ENOUGH. THE SLEDGE HAMMER USER DID NOT, THE HOLDER PUT HIS HAND ON TOP OF STUB AND OTHER EE WAS SWINGING HITTING THE HAND OF THE HOLDER.

EMPLOYEE WAS LOADING OUT TRUCKS WHEN THE SECURITY ALARM WENT OFF. HE CAME DOWN THE STEPS GOING TO SHUT OFF AND SPRAINED HIS ANKLE.

EE WAS CHANGING OUT A CARBON DIOXIDE TANK AT 1:30 PM. AT ABOUT 5 PM, HE FELT HIS BACK TIGHTENING UP.
