Employee was cleaning catwalk and turned sending intense pain to EE's foot/ankle area making it hard to move and walk. Employee has had foot discomfort on left foot for a couple weeks and never told anyone. Employee was seen in ER, was given an injection for pain, and taken out of work for 1 day. EE was diagnosed with tendonitis.
Little finger on left hand got pinched between pry bar and beam.
Employee was lifting a steel plate. The plate slid on the concrete floor at which point the employee had their finger pinched in between the floor and plate.
Employee arrived to work at normal time (6am) and performed normal duties (sitting in crusher shack operating jaw). Around 10 am employee starting having knee pain. Employee stated that EE does not know what caused the pain. Employee went to urgent care and was given prescription medication and taken out of work for 2 days.
Injured worker was team lifting a 60lb sheave when the sheave slipped out of coworker's hands and dropped on IW's right hand. When the sheave was dropped it pinched IW's right hand between the sheave and some steel guarding in the area that resulted in IW receiving 4 stitches to the right ring finger. Injured worker was placed on light duty for 2 weeks.
Employee was assisting with a screen change out during wet, rainy conditions during the shift. Employee went home, showered, went to bed, and woke up around 3am with pain in left eye and went to the ER. ER doc removed a small foreign body from EE's eye. Employee followed up with eye doc who returned EE to work.
While throwing a rock into the jaw to help get a big rock out of the crusher employee's hand was smashed between the rock and the I beam in the front of the opening. This caused the skin to open on left thumb.
Employee was assisting in reinstalling the top spring plate on a Deister screen. While doing so, employee's hand slipped off the plate as the plate was seated and got left pinky finger pinched between the plate gusset and screen gusset. Employee received 7 stitches to close the injury.
While placing a screen panel on the catwalk another screen scraped EE's right forearm causing lacerations, requiring 19 stitches.
Employee was removing a guard on C-13 conveyor in order to change a return roller. He untied the guard and it swung down and struck him on the left foot. This resulted in a puncture wound to his left big toe.
Employee was removing bolts from the large cover for the blend chute, as he was taking the last bolt out it came down and hit his left thumb and cut it. He received 6 stitches.
Re-palletizing stone pinched right index finger when stone moved.
EE was helping another Emp. remove a rock from the portable crusher and while lifting on the rock, it shifted and pinched his thumb between the rock and the apron plate, causing an avulsion to his thumb. Emp. had surgery to repair avulsion to thumb.
The wedges on the back of the portable crusher hopper vibrated out and the hopper sides opened up slightly. EE went to clear the rocks out of the way and after clearing the last rock, the back of the hopper fell down striking him on the top of the foot, fracturing it. The hydraulic cylinder which moves the hopper side up and down was defective.
Mechanic replacing a blown hydraulic hose on a 365 Cat Excavator. Mechanic bled pressure from the pilot control, lossened the hydraulic tank plug, and relieved pressure from the tank prior to maintenance. However, pressure built back up in the line and during the removal of the hose the pressure released, sprayed and contacted the mechanic's eye.
EE was trying to put safety pin in slot of rotor when it loosened up enough for the rotor to slide and pinch his finger between the rotor and the wall of the crusher.
Employee was standing on the take up pulley knocking a stuck rock out, when the belt rolled back approx. 12 inches pinching his feet between the take up pulley and the belt. Resulting in a minor bruise to his foot/ankle.
While installing a pipe section on the plant slurry pump, EE attempted to lift into place a section of pipe. He slipped in the mud & caught his fingers on his left hand, between 2 pieces of pump, resulting in 2 broken fingers on left hand.
Employee was pouring cement. He was only wearing leather workboots so cement/water went through the boots to his feet.
Employee was performing a pre shift walk around inspection on the water truck. He raised the hood to allow for a fluid check. As EE walked around to the other side of the truck he struck his head on the mirror causing a cut at the hairline on his forehead.
WHILE DRIVING THE WATER TRUCK EMPLOYEE DISCOVERED A ROCK THAT FELL OFF A HAUL TRUCK. EMPLOYEE MOVED THE ROCK OFF THE ROAD. ON 5/26/06 HIS BACK WAS REALLY HURTING HIM. SENT TO HOSPITAL.
Changing dragcable on dragline. I was putting cable in the hole and let go of cable. to push more in the hole it came back and hit me in the forhead ouch. I was wearing my hardhat and safety glass, gloves.
EE was cleaning up brush & tree limes from a storm and that had been previously cut to clean fence row. During this process a small twig or branch entered the ear canal and penetrated the ear drum.
EE had just finished greasing one side of our main screen, he was on the ground, attempting to move the ladder to the other side of the screen when it got away from him. Instead of letting the ladder fall over he tried to "save" it. Hence he strained his back muscles trying to slow the ladders descent.
WORK WAS BEING PERFORMED ON TWO DIFFERENT LEVELS IN THE SAME AREA. THE UPPER CREW DROPPED A LARGE SOCKET JUST AS THE LOWER EMPLOYEE DUCKED UNDER THE OPENING. HE WAS STRUCK BY THE SOCKET ON TOP OF HIS HEAD. EMPLOYEE REQUIRED SEVEN SUTURES TO THE TOP OF HIS HEAD.
Employee lifting bearing and housing off of a press when he felt a burning sensation in the back of his head, down his clavicle on his left side. Employee received a cervical radiculitis and was put on work restrictions.
EE WAS ROLLING BEARING HOUSING FOR 8 X 20 SCREENDRIVE. HOUSING FELL TO ONE SIDE AND CAME DOWN ONLEFT HAND, FRACTURING SMALL FINGER.
INJURED WAS INSTALLING NEW V-BELTS ON A SIMPLICITY 5 BY 12 WET SCREEN, ROLLING BELTS ONTO THE SHIEVE, WHEN HIS RIGHT MIDDLE FINGER GOT CAUGHT BETWEEN THE TAUGHT BELTS AND SHIEVE, PINCHING SMASHING THE NAIL OF RIGHT MIDDLE FINGER OF AFFECTED AREA.
