Was removing conveyor belt off welding truck, hit hand against vise, next day hand started hurting. Went to DR took x-ray, would let EE know. They called July 5 and said EE did have piece of metal. They removed it July 6, no stitches, no lost time.
Employee was removing a metal strainer from a pvc line at our Sand plant, when he tried to remove the pvc line from the metal strainer he cut top of left thumb.

EE was putting new brake pads on dump truck when he felt a strain in lower back. Employee went to doctor and was put on light duty work for 4 days.We are looking into this accident. We had a helper with him, he did not report any injury friday before he left work but came in monday morning first thing and reported back injury.
Employee was helping unload motor, motor slipped, caught rt thumb between motor and truck bed, causing brusing to rt thumb. Doctor put rt hand in a brace.

EE WAS HELPING ANOTHER EE REPLACE A STAR PLATE ONTO CANICA CRUSHER. USING A CRANE TO LIFT STAR PLATE (45-50#'S)WHILE UNHOOKING PLATE EE HAD HIS FOOT UNDER PLATE & IT FELL 3-4" ONTO HIS FOOT. CAUSED BRUISED FOOT. EE WAS WEARING HARD TOE SHOES. EE DID NOT GO TO DR UNTIL 4/23/04. EE ON CRUTCHES FOR A WEEK.
EE WAS ROLLING UP TARP ON DUMP TRAILER, TARP WAS HEAVY, HANDLE GOT AWAY FROM EE AND HIT HIS ARM, CAUSING A CONTUSION TO RIGHT ARM REQUIRING HIM TO WEAR A ARM SPLINT.

EE WAS SHOVELING MATERIAL OUT FROM UNDER BINS FOR APPROX. 10 MINUTES WHEN HE FELT PAIN IN HIS LOWER BACK AND DOWN HIS LEG.
EE WAS AT THE OLD SHAKER SCREEN, (NOT IN USE) TURNING A SHEAVE TOP REMOVE BELTS. THE COUNTER WEIGHT SHIFTED AND CAUGHT HIS ARM. THERE WAS A PIECE OF SPOT WELD ON THE METAL WHERE HIS ARM WAS AN D WHEN EE WENT TO PULL HIS ARM OUT IT CAUSED A LACERATION TO HIS RIGHT FOREARM.
