Struck by falling object

EE shut down crushing plant to remove a drill bit stuck in the jaw crusher. He was using a torch to cut off drill bit when the bit flew up into his chest & face. He was hospitalized and died on 1/17/15.

Large rock was on rock breaker hydraulic line, went to roll rock off line when it rolled catching thumb between rock and frame of breaker causing a small laceration.

Struck by falling object

Employee was changing out motor on #1 Conveyor. EE was using a pry bar to help lift motor when bar slipped letting motor fall on tip of finger causing fingernail to pop off and cutting tip of finger.

Miner was working on wiper underneath the screen, wrench slipped and hit elbow. Miner was taken to the ER, was referred to orthopedic, where he was diagnosis with contusion left elbow/triceps tendon.

Employee was replacing hydraulic line on Terex loader. When taking bolts loose, the guard that covered the line fell on his foot. Employee reported accident to manager, refused to go to doctor, has missed two days of work. On 11/30/07, I made a doctor's appointment for him. He said he would go, but didn't show up for the appointment.

EE WAS GETTING OUT OF EQUIPMENT, STARTED WALKING UP EMBANKMENT, HE SLIPPED ON ROCK THAT HAD SAND ON IT. EE HAD SCRAPES ON BACK, LEFT ARM, AND LEFT LEG. BACK WAS HURTING PRETTY BAD. EE SAID HE DID NOT NEED TO GO TO DR, AND REMAINED AT WAORK FOR THE REMAINDER OF THE DAY.

Struck against stationary object

WAS BENT OVER GREASING CONVEYER, HARD HAT FELL OFF, WHEN HE RAISED UP, HE HIT HIS HEAD CAUSING A LACERATION REQUIRING STITCHES.
