Employee was moving stone on the saw when EE felt a pain in EE's right side, this is during the week of 11/18/2024. The pain got worse as time went on and EE decided to be seen on 11/22/2024. EE was diagnosed with musculoskeletal strain/sprain.

Reached between two pieces of stone to retrieve steel plate and one stone shifted pinching finger.
Employee was moving a steel cable.
As employee was removing plug wedges from stone, the stone opened up and the employee struck his forearm on the edge of the stone, causing a laceration which required sutures to close.
Employee reached out to pull fall block towards him and block was still lowering, which pulled left hand into fall block sheave, causing amputation to four fingers on left hand. Fingers were to badly damaged to be reattached.
While putting a drill rod in the shank - the shank was turning and the rod caught his hand and twisted it causing ligament strain to wrist, elbow and shoulder.
A piece of granite rolled onto employee's foot and struck his instep when a ladder was moved causing fractures to four metatarsels.
While pulling a suction hose out of the water, the hose twisted and his hand caught in the check valve. He felt a "pop" in his left hand at that time. He did not go to the doctor until 10/02/2006. Inflamed tendons in the palm of the left hand.

Employee lifted socket on the end of a steel cable and felt pain in groin area.
EMPLOYEE FELT PAIN IN HIS BACK WHEN HE TRIED TO PULL A TRANSMISSION JACK OUT OF A STORAGE AREA.
EMPLOYEE WAS PULLING ON A AIR 4" HOSE AND FELT A BURNING SENSATION IN HIS LEFT SIDE. HE DID NOT GO TO THE DR UNTIL 12-1-03.
THE EE WAS PRYING A PIECE OF GROUT AND FELT PAIN IN GROIN AREA.
WHILE LIFTING A PIECE OF DRILL STEEL EMPLOYEE FELT PAIN IN HIS BACK.
AS ANOTHER EE PUSHED A STONE OVER A SHORT LEDGE, THE STONE CAUGHT AND ROLLED TOWARDS AN EMPLOYEE AND THE STONE STRUCK THE EMPLOYEE IN THE KNEE AREA OF HIS LEFT LEG.
EE WAS PUSHING A PIECE OF WOOD BLOCKING UNDER A GRANITE BLOCK WITH A LENGTH OF STEEL PIPE. HE FELT PAIN IN HIS LOWER BACK.
EE LIFTED A SMALL STEEL RACK (ABOUT 30 LB.) AND TWISTED HIS BODY AS HE PUT IT ON A SHELF. HE FELT PAIN IN THE MIDDLE OF HIS BACK AFTERWARDS.
BENT OVER TO LIFT A WIRE CABLE.
EMPLOYEEWAS PULLING ON A 4" AIR HOSE WHEN HE FELT PAIN IN MIDDLE OF HIS BACK ON THE LEFT SIDE.
AS THE EE WAS MOVING A LARGE DRILLING MACHINE WHEN A HOSE CAUGHT ON ANOTHER HOSE. AS HE PULLED ON HOSE A METAL PIPE ATTACHED TO THE SECOND HOSE CAME OUT OF A CHANNEL AND STRUCK HIM ON THE SIDE OF HIS HEAD JUST BELOW HIS HARD HAT.
AS EE WAS MOVING RAILING SECTIONS ONE FELL OVER & FELL ON HIS FOOT
EE WAS PULLING ON A 3" AIR HOSE WHEN HE FELT PAIN IN HIS BACK.
AS EE WAS QUARRYING STONE, HE FELT PAIN IN GROIN AREA.
LETTING A JACKHAMMER DOWN OVER EDGE OF STONE.
EE PICKED UP AN 18' PIECE OF DRILL STEEL. AS HE TURNED TO WALK AWAY HE FELT PAIN IN GROIN AREA.
WHILE MOVING LARGE METAL DOOR ON BOILER THE EMPLOYEE FELT PAIN IN LOWER BACK.
WHILE CHAINING A PIECE OF STONE EMPLOYEE'S FINGER STRUCK A SHARP PIECE OF STONE.
