while inflating a bag to separate granite piece from wall the bag blew up causing the rope and hose made contact with EE's hand and caused an injury

Employee was moving stone on the saw when EE felt a pain in EE's right side, this is during the week of 11/18/2024. The pain got worse as time went on and EE decided to be seen on 11/22/2024. EE was diagnosed with musculoskeletal strain/sprain.

On 9/5/2024 While unloading a block employee alleges that employee was jolted in the cab of the loader, & employee jammed wrist into the steering joystick causing pain. Employee did not report this accident to supervisor, nor did EE associate an injury with an incident. On 9/5 employee reported having aggravation in wrist due to carpal tunnel. On 10/7 EE asked to be checked out

On 5/29/2024 Lifting deadman link on JL6600S manlift and felt a bee sting in EE's arm, Charlie horse feeling in right arm and bicep. On 5/30/2024 the area where EE felt discomfort turned black and blue.

Conducting secondary breakage.; when picking wedges the stone shifted causing the employee to lose balance and fall from the stone. The fall arrest engaged and spun EE around where EE's upper arm caught on the edge of the block dislocating EE's shoulder

EE was holding a pipe for another EE who was hitting the end of the pipe with a sledge hammer. The employee who was hitting the pipe, missed the pipe and the hammer EE was using struck the left thumb and wrist area, of the EE holding the pipe, causing a hand injury.

Spotting hole with Plug drill and the bit slipped lacerating a finger

Walking stepped on small stone and rolled EE's ankle

Swinging Sledge Hammer and hurt shoulder

Bent over to cut diamond wire and threw EE's back out

Trying to move a stuck lever on a Double drill and hit it with the heel of hand and it broke loose but EE caught thumb on the rotation lever and bent thumb over backwards.

Laceration to (R) middle finger while unloading hydro bags. Gloves were not worn.

Employee was lifting a plug drill (About 25 lbs.) when he felt a pain in his lower back.

Employee tripped and fell on his right side as he stepped from a ladder onto a granite block.

5 employees were setting a staging - when the staging was set down, one employees foot was underneath the edge, causing a crush type injury to the foot.

Walking from one hoist house to another, he slipped and fell. Knee pain.

Reached between two pieces of stone to retrieve steel plate and one stone shifted pinching finger.

Employee was moving a steel cable.

Employee reached out to pull fall block towards him and block was still lowering, which pulled left hand into fall block sheave, causing amputation to four fingers on left hand. Fingers were to badly damaged to be reattached.

AS HE WAS DRILLING A NOTCH IN A SAW BLOCK WITH A JACKHAMMER THE ROD SLIPPED OUT OF THE NOTCH AND PINCHED HIS FINGER BETWEEN THE JACKHAMMER AND A METAL HOSE FITTING.

Employee was helping take a twist out of a fall cable on derrick, when he slipped and struck a glut set up in next line with chest area, causing a fracture to 10th rib on right side.

Twisted knee walking on grout. Did not go to the doctor until 12/11/2007 because his knee was sore.

A piece of granite rolled onto employee's foot and struck his instep when a ladder was moved causing fractures to four metatarsels.

Employee was baring a piece of stone when the bar slipped and he jarred his back. Thoracic sprain.

Setting up a line to break with shims and wedges. The shim had a sharp edge and cut employee's hand as he set it in drill hole.

He stepped back and stepped on the edge of a hammer which came up and the handle struck the back of his right leg. Partial rupture of achilles tendon.

Employee was shoveling snow to clear work area then he felt pain in his arm later.

Employee lifted socket on the end of a steel cable and felt pain in groin area.

EMPLOYEE STEPPED ON A STONE. DID NOT REPORT INJURY UNTIL 5/24/2004.

EMPLOYEE STRUCK PLUG WEDGE WITH SLEDGE HAMMER TO TIGHTEN WEDGE. WHEN HE DID THIS THE WEDGE POPPED OUT AND STRUCK HIM IN THE CHEEK. SMALL LACERATION TO CHEEK. THREE SUTURES.

EMPLOYEE WAS STANDING STILL WHEN HIS BACK BEGAN TO ACHE.

AS ANOTHER EE PUSHED A STONE OVER A SHORT LEDGE, THE STONE CAUGHT AND ROLLED TOWARDS AN EMPLOYEE AND THE STONE STRUCK THE EMPLOYEE IN THE KNEE AREA OF HIS LEFT LEG.

EMPLOYEE WAS WALKING ON UNEVEN GROUND. HE FELL DOWN WITH HIS WEIGHT ON OUTSTRETCHED ARM, RESULTING IN POSSIBLE ROTATOR CUFF TEAR. *ORIGINAL INJURY ON 6/12 - EMPLOYEE DID NOT GO TO DOCTOR RIGHT AWAY. BEGAN LOSING TIME 7/10/02.

EE WAS PUSHING A PIECE OF WOOD BLOCKING UNDER A GRANITE BLOCK WITH A LENGTH OF STEEL PIPE. HE FELT PAIN IN HIS LOWER BACK.

BENT OVER TO LIFT A WIRE CABLE.

EMPLOYEEWAS PULLING ON A 4"" AIR HOSE WHEN HE FELT PAIN IN MIDDLE OF HIS BACK ON THE LEFT SIDE.

AS THE EE WAS MOVING A LARGE DRILLING MACHINE WHEN A HOSE CAUGHT ON ANOTHER HOSE. AS HE PULLED ON HOSE A METAL PIPE ATTACHED TO THE SECOND HOSE CAME OUT OF A CHANNEL AND STRUCK HIM ON THE SIDE OF HIS HEAD JUST BELOW HIS HARD HAT.

WALKING TO DRILL EQUIPMENT HE SLIPPED WHEN HE PICKED HIS FOOT UP OVER AN AIRHOSE. SPRAINED RIGHT ANKLE.

EE WENT GROUND A SAFETY RAILING WITHOUT FALL ARREST TO DISCONNECT AIR LINE. HE FELL OFF LEDGE AND STRUCK KNEE ON GRANITE SURFACE.

AS EE WAS MOVING RAILING SECTIONS ONE FELL OVER & FELL ON HIS FOOT

EE WAS PULLING ON A 3"" AIR HOSE WHEN HE FELT PAIN IN HIS BACK.

WRIST BECAME SORE SHARPENING DRILL BITS.

WHILE WALKING EE TWISTED RIGHT KNEE. NO LOST TIME OR RESTRICTED DUTY AT TIME OF INJURY LOST TIMEBEGAN 3/2001

EE PICKED UP AN 18' PIECE OF DRILL STEEL. AS HE TURNED TO WALK AWAY HE FELT PAIN IN GROIN AREA.

EE SHARPENS DRILL BITS. HE FEELS WRISTS HAVE BEEN GETTING SORE BECAUSE OF THIS WORK.

WHILE MOVING LARGE METAL DOOR ON BOILER THE EMPLOYEE FELT PAIN IN LOWER BACK.

WHILE CHAINING A PIECE OF STONE EMPLOYEE'S FINGER STRUCK A SHARP PIECE OF STONE.

EE WAS CLIMBING A SHORT LADDER TO HOOK UP AIR HOSE. THE LADDER SHIFTED, EE STRUCK KNEE AS HE SLIPPED OFF CLEAT. STRAINED LIGAMENT IN LEFT KNEE WHEN HE TWISTED.
