EMPLOYEE WAS CUTTING HARDWOOD WEDGES ON A BENCH SAW. PIECE OF WOOD WAS KICKED BACK STRIKING EMPLOYEE ON NOSE.
EE LIFTED A SMALL STEEL RACK (ABOUT 30 LB.) AND TWISTED HIS BODY AS HE PUT IT ON A SHELF. HE FELT PAIN IN THE MIDDLE OF HIS BACK AFTERWARDS.

AS EE WAS BREAKING A PIECE OF DIMENSION STONE A HIDDEN SEAM OPENED AND A PIECE OF STONE FELL ANDHIT HIS FOOT.
AS EMPLOYEE WAS BREAKING A PIECE OF STONE WITH PLUG WEDGES THE STONE OPENED UP AND FELL AGAINST EMPLOYEES SHIN CAUSING SCRAPES ALONG HIS RIGHT SHIN.
GROUT WEDGED IN DUMP BODY AS HE WAS DUMPING IN DIMENSION YARD. THIS STOOD TRUCK ON END, GROUT THEN DISLODGED SENDING TRUCK TO GROUND. EE WAS WEARING HIS SEAT BELT BUT STILL SUSTAINED FRACTURE TO SPINE WHEN VEHICLE CAME DOWN.
EE WAS INSTALLING GEAR BOX ON DRILL USING AN OVERHEAD CRANE TO LIFT GEAR BOX. AS GEAR BOX CONTACTED DRILL MAST, THE CHAIN WENT SLACK AND GEAR BOX TOPPLED OVER. AS IT FELL, IT PUT WEIGHT BACK O N CHAIN AND EE''S FINGERS WERE CAUGHT BETWEEN CHAIN AND GEAR BOX. PARTIAL AMPUTATION OF RING AND SMALL FINGER OF LEFT HAND.
