Employee was changing out wear liner in crusher. One of the liners that EE was handling slipped out of EE's hand and fell onto the top of EE's left foot. Employee was wearing approved steel/composite toe boots. Liner landed on laces area of boot not protected by steel/composite material.

Employee was refueling the fuel truck, as EE was putting the fuel nozzle on the back door of the truck the door closed on EE's finger. Smashing the nail and the tip of the finger.

EE was working to rebuild the tail frame of CONV6008. They had cut and removed the old framework, dragging it away from the tail area. While walking across this area, a sharp corner of the old frame caught their pant leg, causing them to stumble into the frame, striking the sharp corner with the inside of their right shin resulting in a laceration.

EE was removing an idler from the jaw belt, when EE pinched ring finger between the idler and conveyor frame, resulting in a laceration. First aid was provided and the supervisor was notified. That evening the wound was still bleeding and EE went to get stitches.

EE performed various heavy labor activities throughout the shift. The last activity was removing a 4'x1'x.5' sheet of metal from overhead return head chute. A chain come-along was used to lift the sheet, then EE and a co-worker lifted the sheet from the chute top to the conveyor. EE texted co-worker that evening stating back was hurting.

EE picked up a 1" ounce of steel off the back of a service truck to put into a scrap bin. The steel was slippery and it slipped from EE's hands and landed on EE's left foot right behind the steel toe.

Employee was flopping a gate on a chute. Another employee pulled on the gate to help free it up. When he pulled on the gate, the handle on the outside of the chute caught the employee's hand and pinched it between the handle and the handle lock.
