Employee was changing out wear liner in crusher. One of the liners that EE was handling slipped out of EE's hand and fell onto the top of EE's left foot. Employee was wearing approved steel/composite toe boots. Liner landed on laces area of boot not protected by steel/composite material.

Employee was refueling the fuel truck, as EE was putting the fuel nozzle on the back door of the truck the door closed on EE's finger. Smashing the nail and the tip of the finger.
