While working on welding plate lid into chute, plate lid slipped off and hit R ring finger of employee causing a partial amputation between plate and angle iron. After evaluation surgeon decided to complete amputation at 1st knuckle and close up amputation.

Over-exertion in lifting objects

EE performed various heavy labor activities throughout the shift. The last activity was removing a 4'x1'x.5' sheet of metal from overhead return head chute. A chain come-along was used to lift the sheet, then EE and a co-worker lifted the sheet from the chute top to the conveyor. EE texted co-worker that evening stating back was hurting.

Struck by falling object

EE picked up a 1"" ounce of steel off the back of a service truck to put into a scrap bin. The steel was slippery and it slipped from EE's hands and landed on EE's left foot right behind the steel toe.

Struck against stationary object

Smashed left hand against a 45 degree pcs. of metal rain drip edge, cutting the pinky finger badly & breaking the bone, also cut the ring finger on the same hand

EE was trying to jar some rocks loose in the jaw crusher and caught hand between a chain and the hand rail.

Employee was flopping a gate on a chute. Another employee pulled on the gate to help free it up. When he pulled on the gate, the handle on the outside of the chute caught the employee's hand and pinched it between the handle and the handle lock.

9-26-02 THURS AM, IN OFFICE AREA. THOUGHT HE HAD A STICKER OR SOMETHING IN HIS PANTS LEG AT KNEE. NOTHING SAID AT THE TIME, BUT KNEE HURT DURING THE DAY AND BY NEXT DAY WAS SWOLLEN AND RED. SO UGHT DR ATTENTION NEXT DR INFOR EE IT WAS PROBABLY A SPIDER BITE.

THE VICTIM HAD SPENT TWO MINUTES FREEING A PARTIAL JAM W AN IMPACT CRUSHER WITH A BAR AND FEW MINUTES LATER WHILE STILL ON THE CRUSHER WORK PLATFORM PREPARING TO GO DOWN THE STEPS AND BACK TO HIS LOADER, HE SUFFERED A MASSIVE HEART ATTACK AND WAS PROUNCED DECEASED AT THE HOSPTIAL.

EMPLOYEE WAS CLEANING HIS WINDOWS ON THE LOADER, STEPPED AROUND TO THE FRONT OF THE LOADER OFF THE WALKWAY AND PULLED A MUSCLE IN HIS BACK.

WHILE SERVICING HIS EQUIPMENT IN THE PREDAWN TIMEFRAME, THE INJURED WALKED 9 FOOT AWAY FROM THE DRILL AND STEPPED OFF THE EDGE, SLIDING 6 FT AND THEN FALLING 18 FT. BROKE HIS RIGHT WRIST LEFT FOREARM, AND HIS JAW. HE WALKED OUT OF THE PIT TO THE SURFACE LEVEL, WAS LIFEHIGHTED TO THE HOSPITAL AS A PRECAUTION.
