Employee had removed material from the Niro by raking it out of the Niro door. Employee was attempting to shut the Niro door by kicking it and sustained a knee injury. Kneecap was dislocated and employee was treated at a clinic. The kneecap was manipulated back into place and employee was put in a flexible brace.

While swapping and cleaning Niro filters per schedule, employee was using improper P.P.E. and not following proper procedures, exposing skin to potential hot slurry spillage. EE opened the filter lid before closing off inlet/outlet valves resulting in hot slurry to land on the back of EE's neck causing a burn.

EE and co-worker were relocating a 32ft. extension ladder. When they stood it up straight, the ladder retracted suddenly with the upper half sliding suddenly downward and struck EE's left hand, causing a laceration to EE's left-hand ring finger and pinkie finger.

Employee was locking the deck back in place after changing screens on 3-3 green screen using the manual locking bar. Upon applying downward pressure using EE's right arm the employee felt a strain in EE's pectoral muscle.

Employee replaced caustic pump 4150, then handled tools that had come in contact with caustic soda & rubbed left eye with fingers. Bottom eyelid became irritated so employee flushed eye several times, several minutes each time & took OTC anti-inflammatory meds & applied ice pack for swelling. Saw optometrist 08/19/22.

Employee was cleaning up around Kiln back spill hopper. Material falling to floor got behind EE's safety glasses and created irritation in and around EE's eyes. EE rubbed eyes with EE's hands, later infection appeared specifically around EE's right eye. EE sought medical attention at Macon Occupational and was referred to Georgia Eye Care Center, who prescribed an oral antibiotic.

Washing off screens on Line 2 blunger with pressurized hose. A splatter of slurry went in eye, caused a burn to the cornea. Not reporting to supervisor in a timely manner and due to possible lack of eye protection and lack of proper flushing /cleaning of eye after accidental exposure. Lack of involvement of first aid trained/qualified supervisor.

Employee was cleaning a plugged lance and positioning lance to install back into the niro hit a light fixture above. Hitting the fixture with the lance caused the globe/dome to break and fell on the back of the worker therefore, injuring EE. PPE required for the task was used. The worker received a laceration on the back and right arm.

Employee was helping with a screen change. Screen was stuck inside screener frame. When EE grabbed the screen, finger got caught between the screen and the frame, causing a laceration on the middle finger of the left hand. Sutures were required. No lost time. Worked 3 days with lifting restriction.

Employee was fitting a 10" 90 degree elbow in preparation to be welded in place. Clamps holding the 10" fitting in place released and the fitting fell and struck employee on the chin. Employee suffered a laceration to his chin requiring stitches.

Employee was replacing broken PVC pipe. He had left a can of PVC cleaner on top of ladder when he climbed down to cut piece of pipe. As he was climbing back up, the can fell and struck his upper lip, causing a laceration that required 2 sutures.

Employee was installing section of tube conveyor. One end was bolted, other end was supported by crane and bolts in slotted holes. During alignment process, conveyor end slipped off of bolts and pinned employee's left thumb between conveyor flange and support.

Laceration to pinky finger (Fifth Finger)- Left hand requiring stiches. Employee was knocking off lances on Niro 7. When closing the door on Tank 6 on the burner side, the door pulled out of his hand due to the negative and smashed his little finger on his left hand. Employee did have on all of his PPE at the time of incident.

Employee was removing heat exchanger from shelf. It was stuck to another plate. He put his hand between them to get them unstuck, and as it came free it slid down his arm and cut the top of his left wrist. Required sutures. Returned to work with no lost time.

Clearing material from inside of the niro lance after shutdown to prevent the lance from becoming plugged during startup. The operator was raising and lowering a lance to remove buildup. While lowering the lance back into the niro the operator did not remove his left hand from underneath the lance collar and smashed his left index finger.

Employee was cleaning final filters on Niro II, all his PPE on. After closing off 3-way ball valves, he cracked open the filter & the pressure knocked if off and sprayed into the filter housing. Slurry bounced back & hit left shoulder & side of face shield, causing hot slurry to run down under his apron.

While working on a stalled bag house screw, employee placed his left hand on the belt. His fingers were pulled into the pulley and caught between it and the drive belts. The tip of his index finger was severed, middle finger was broken (tendon damaged) and ring finger slightly lacerated.

Working in A103 injury removed gloves. Gloves soaked. After removing gloves he started rolling up a hose.Pulled hose with right and grabbed with his left hand & ran his fingers over the hose clamp which caused a cut to his left middle and ring finger. After shift he went to local emergency room and the middle left finger received stiches with no limitations.

Employee was lifting the lid on a railcar that he was loading. He felt a pull in his back, but didn't feel it necessary to see a doctor at that time. EE continued to feel soreness in lower back and saw the company doctor on 9/3/08. Dr. gave employee a lifting restriction of 15 lbs. until next appointment.

Employee was unloading a box car of Super Sacks. He was lifting the straps to enable the forks on the forklift to go through the straps so the bag could be picked up. When he lifted the straps closest to the wall of the box carl, he advised the forklift operator to come forward and his finger was pinched between the fork and the wall of the box car.

Employee was closing a railcar discharge gate. The compartment gate was stuck. He put pressure on the gate and it broke free causing him to lurch forward and overextend his right arm. Resulted in right shoulder strain. INCIDENT ORIGINALLY CLASSIFIED AS NEAR MISS UNTIL EMPLOYEE REPORTED ON MONDAY, JULY 23 THAT HE NEEDED HIS SHOULDER CHECKED.

Employee dumped a dirt hopper in plant 2 warehouse. After dumping hopper, he got off his forklift and put hopper back in locked position. When he did this he smashed his left ring finger between the forklift mask and Roura Hopper.

Employees were checking baghouse for damaged bags. One employee slipped while moving a bag and cage full of material and the cage fell and cut the head of the other employee who was kneeling. This accident occurred in an area of a baghouse that is hot and dusty with trip hazards and pinch points.

EE WAS INSPECTING TOP FLOP GATE FOR AN AIR LEAK WHEN THE CYLINDER ACTUATED THUS PINCHING HIS RT.INDEX FINGER BETWEEN THE CYLINDER BODY AND THE FLOP GATE ARM. DR. PUT 3 STITCHES IN HIS FINGER & PLACED HIM ON RESTRICTED DUTY FOR 7-10 DAYS.
