Contact with hot objects or substances

Employee was working on the roof of an office building. Taat roof was approx. 12' from the ground. A hi-power electrical line approx. 15' above the roof ran parrallel to the bldg. He was installing a piece of metal trim when he was hit by hi voltage. His clothing caught fire causing severe burns.

Employee was unloading a box car of Super Sacks. He was lifting the straps to enable the forks on the forklift to go through the straps so the bag could be picked up. When he lifted the straps closest to the wall of the box carl, he advised the forklift operator to come forward and his finger was pinched between the fork and the wall of the box car.

Employee dumped a dirt hopper in plant 2 warehouse. After dumping hopper, he got off his forklift and put hopper back in locked position. When he did this he smashed his left ring finger between the forklift mask and Roura Hopper.

An employee claims he was bitten by a spider on 10-10-2006. The affected area developed a staph infection after several days. The employee was admitted into the hospital on October 14th, 2006.

Struck against a moving object

While pushing material, the front end loader started bouncing and then I felt a sharp pain. It wasn't till an hour & a half later after I got home that my back starting hurting.

Accident occured as EE was bent over under railcar in attempt to pry a stop chock away from the wheel of a rail car. Using a pry bar (12-15 lbs) swung the bar twisting his upper body and back.

While opening a bulk bag using as a trash bag, cut finger with a box cutter. Cut middle finger wihile holding bulk bag, as he was cutting the bag razor slipped and contacted finger.

2 MAINTENANCE SPECIALISTS WERE CLOSING AN INSPECTION DOOR ON THE FIRING HOOD OF A ROTARY KILN. THE DOOR WAS DIFFICULT TO CLOSE & THE EE'S HAD TO BEND OVER & LEAN AGAINST THE DOOR TO GET IT TO CLOSE. THE EE STRAINED HIS LOWER BACK WHILE LEANING AGAINST THE DOOR.

EMPLOYEE WAS CLEANING THE FEED BOX OF A VIBRATORY SCREEN UNIT WITH A LENGTH OF FLATBAR AS A SCRAPER. THE FLATBAR BECAME LODGED ON THE FEED BOX, SLIPPED AND CUT THE EMPLOYEE'S LEFT HAND.

EE OBSERVED AN OBSTRUCTION INT HE DISCHARGE OF A 3 INCH HELICAL SCREW FEEDER. WITHT HE SCREW ENERGIZED, HE REMOVED THE EXPANDED METAL GUARD AND PLACED HIS HAND INTO THE FEEDER TO REMOVE THE OB STRUCTION. BOLTS ON THE SCREW PULLED HIS HAND BENEATH THE SCREW CAUSING LACERATIONS AND 2 FRACTURES.

EE WAS DESCENDING A STAIRWAY ONTO A LANDING. HE RELEASED HIS HAND FROM THE HANDRAIL AND TWISTED HIS ANKLE ON THE LANDING.

2 EXPERIENCED & SKILLED MAINT SPECIALISTS WERE REMOVING A STATIONARY SEAL PLATE FROM A ROTARY DRYER. 1 MAN FREED THE PLATE FROM ITS RETAINING BOLT IT ROTATED ON ITS PIVOT & CAUGHT THE LEFT H AND OF THE OTHER MAN BEWEEEN SEAL PLATES THE PLATE THEN CAME INTO BRIEF CONTACT WITH THE ROTATING DRYER FUTHER INJURING HIS HAND RESULTING INMULTIPLE FRACTURES & LACERATIONS NO DAMAGE OR IM
