EE and co-worker were relocating a 32ft. extension ladder. When they stood it up straight, the ladder retracted suddenly with the upper half sliding suddenly downward and struck EE's left hand, causing a laceration to EE's left-hand ring finger and pinkie finger.

EE was walking around a bag and tripped on the pallet the bag was sitting on, causing EE to fall on the ground and land on EE's right shoulder.

Employee replaced caustic pump 4150, then handled tools that had come in contact with caustic soda & rubbed left eye with fingers. Bottom eyelid became irritated so employee flushed eye several times, several minutes each time & took OTC anti-inflammatory meds & applied ice pack for swelling. Saw optometrist 08/19/22.
Employee was dislodging material from dryer transition using a sledgehammer. EE struck the flange for the access door and smashed the tip of middle finger on right hand. As a result, EE has a ""displaced fracture of distal phalanx of right middle finger, initial encounter for closed fracture"".
Employee was cleaning up around Kiln back spill hopper. Material falling to floor got behind EE's safety glasses and created irritation in and around EE's eyes. EE rubbed eyes with EE's hands, later infection appeared specifically around EE's right eye. EE sought medical attention at Macon Occupational and was referred to Georgia Eye Care Center, who prescribed an oral antibiotic.

Employee was swapping valve from bagging bin to truck bin. The truck valve would not open, and while troubleshooting and concentrating on the truck valve EE placed left hand on part of the bagging valve. When the bagging valve actuated it mashed EE's left middle finger. First aid was administered at the plant, then EE was taken to a physician where EE received stitches.
Miner (Service Technician) with amp meter inside live 480V Electrical Panel performing maintenance on air compressor without proper PPE. Unknown short across bottom of motor starter causing arc flash while miner was in front of electrical cabinet. Miner received burns to hands and flash burns to eyes from arc flash.

Employee was checking the atmosphere in a confined space in #1 Kiln. While ascending a ladder, left foot hit a leaf seal, causing employee to lose balance. Employee then fell backwards and hit the left side of their back on a steel frame for the kiln head.

Employee had cleaned out slide gate around 8:30 a.m. and then again around 11:30 a.m. After cleaning the employee took lunch break, ate lunch and drank a bottle of water. Afterwards was sitting listening to radio when employee passed out and 911 was called.

Employee was using a grinder to remove small section of the belt feed chute. He was using all required PPE & a properly maintained grinder. As the employee was cutting the metal, the 6"" blade became lodged & jumped out of the employee's hands. The resulting loss of contact allowed the grinder to contact the employee's left index finger.

EE entered the uniform locker area located in the employee break room. He dropped his dirty uniforms into the collection receptacle and as he turned to walk away, he tripped over a mounted floor bench. As he fell to the floor, he extended his arms to catch himself and fell onto his elbows. After the fall, the employee's left shoulder was in pain and he had limited mobility.
The employee cut his right leg with a utility knife. He was cutting towards himself when the knife slipped off the hose and into his leg. He is on blood thinner therefore extra precautions were taken to control the bleeding. He received stitches at the hospital and was released without restrictions.

Employee was working on a bucket elevator. While walking up steps to access welder, he tripped and fell resulting in laceration above his left elbow.
While working on a stalled bag house screw, employee placed his left hand on the belt. His fingers were pulled into the pulley and caught between it and the drive belts. The tip of his index finger was severed, middle finger was broken (tendon damaged) and ring finger slightly lacerated.

Employee was installing an over-running clutch on the ball mill main drive. While putting the sprocket on the over-running clutch, part of the clutch came apart and fell on his left foot causing a fracture.
Employee was trouble shooting a power control panel using a voltage meter rated for a maximum of 600 volts. He used it to check voltage that was 2,080 volts. An arc flash occurred which resulted in second degree burns from his right elbow to his wrist and third degree burns from his wrist to the tips of his fingers. He also has some second degree facial burns.

Employee, along with two other employees, was hoisting a steel beam and steel plate up to one of plant top levels. While attempting to pull beam & plate across handrail to stage up for work, the load shifted & his fingers got pinched between the beam and plate.

Employee was closing a railcar discharge gate. The compartment gate was stuck. He put pressure on the gate and it broke free causing him to lurch forward and overextend his right arm. Resulted in right shoulder strain. INCIDENT ORIGINALLY CLASSIFIED AS NEAR MISS UNTIL EMPLOYEE REPORTED ON MONDAY, JULY 23 THAT HE NEEDED HIS SHOULDER CHECKED.
Employee was attempting to remove screw conveyor drive shaft with sledge hammer - handle cracked and caused him to strain his lower back.
Employee was loading starch hoppers from normal work platform, which had been pushed 1 foot from normal location. He turned to address another employee and stepped in gap where platform normally would be, causing him to fall off platform, strking left leg and arm on a trash hopper. Incident reclassified as a recordable when medical attention was sought.
Employee was hurrying from Plant to Loadout Shack in driving rainstorm. He had his head down to keep rain off his face and ran into the backguard of a ladder on a re-work stand by Silo 2-1. He hit his head, causing hard hat to push into skin, cutting his left ear.

Employee used a bench grinder to smooth up a weld on a clip that went on an auto sampler. While grinding, a foreign object hit his cheek, went under his safety glasses and landed in his left eye. He was smoothing it to prevent anyone from being injured on the sharp edges. Did not think to use a face shield as he was only doing a minimal amount of grinding.
EE stepped out of kiln between forms and caught his leg on a protruding sharp object cutting his leg 2"" above the knee.
Employees were checking baghouse for damaged bags. One employee slipped while moving a bag and cage full of material and the cage fell and cut the head of the other employee who was kneeling. This accident occurred in an area of a baghouse that is hot and dusty with trip hazards and pinch points.

Employee was standing in flat bed of company truck & lifting up on a crate so forklift operator could get forks under load to remove it from truck bed. The wooden crate board he was grasping broke when lifted, causing employee to fall backward over the side of flatbed, catching his right foot temporarily under the load, before falling to the ground and bruising his right knee.

EE WAS MOVING A PIPE SPOOL PIECE SO THAT A FIT AND WELD COULD BE MADE. IN THE PROCESS OF MOVING, ONE END OF THE SPOOL PIECE WENT INTO A SMALL OPENING OF THE NEW VESSEL AND EE TRIED TO CATCH AN D KEEP FROM FALLING.
EE WAS MOVING A PIECE OF STRUCTURAL STEEL WHICH WAS BEING PLACED ONT HE FLOOR TO INSTALL. EE PLACED FOOT ON TOP OF BEAM FLANGE TO HELP STABLE IT. EE DID NOT REALIZE THAT PIECE WAS BEING HOISTE D AND FOOT WAS CAUGHT BETWEEN BEAM AND HANDRAIL.
