While loading the railcar, EE went to raise the access platform so the railcar door could clear the platform. EE grabbed the platform with right hand and pulled up to lift it. EE felt a pop in right elbow. Diagnosed with tennis elbow.

While lowering the new housing tube into position, the employee's left index finger was smashed between the tube and the side sheet of the vibrating screen machine. Finger was lacerated but not fractured. six stitches.

While unplugging a chute, a piece of screen cloth had to be lifted & supported. The material pushed the screen cloth off, striking EE's ear.

While installing saddle liner in 5 1/2 ft cone crusher, the part became wedged. While freeing the part it rolled back, pinning employee's LT hand between the part & the main frame of the crusher.

Changing media panels in the 25lb 10x24 screen deck. While trying to get panel in place, EE felt left shoulder discomfort. EE did not think it was serious, continued working. The following day (Sunday) EE had pain & minimal ROM. EE went to the ER and then text plant manager.

While changing out roller, they reached inside framework to move the roller, roller stuck on conveyor belt. They pulled hard, roller broke free, caused hand to contact with sharp metal on old conveyor truss framework.

Cleaning catch box, a rock was jammed inside. Another employee threw a rock down into the hole while the injured employee reached down to get a different rock. Rock struck injured employee's hand.

A log got stuck in the dredge hopper. EE got supports to block the grizzly from moving. EE was positioning the support by reaching through the handrails. The support slipped, the grizzly kept descending and struck EE's hardhat. EE pulled head back, cutting nose on the handrail.

Replacing a return roller. While repositioning the roller, it slipped off the pin, fell to the frame, striking finger. Right 4th finger distal phalanx fracture.

The door on side of Stedmon Mill was frozen. Employee was trying to pry it open. He lost grip and the door sprung back catching tip of his left little finger between the door frame and the door. The door is constructed of 3/4" steel plate. The frame is also steel. Employee received a fracture to his left pinky finger.

Employee was the Stedmon plant operator for the day. Employee was opening the feed gate on the feed hopper when it came down unexpectedly catching employee's left ring finger between the handle and a guard. Employee's left ring finger was dislocated. Was taken to ER. Is restricted. No lost time.

Employee was installing a chute on the #2 crusher. The chute swung back and employee's left hand and wrist was crushed. Employee was taken to the ER via ambulance. Was released. Was seen by an Orthopedic Specialist. Was given a splint and is restricted.

Employee was opening a crusher. While adjusting the crusher, the jack fell over and employee reached out to grab it and then the head of the jack retracted catching the employee's right fingertips. He received fractured/crushed right 3 fingers. The right forefinger could not be repaired and the tip was removed. Employee returned to restricted work on 6-23-12.

Employee was doing clean-up. Employee was shoveling spilled material around the plant and felt pain and numbness in his wrists. Employee claims he had felt the pain several days before but did not report it until 9-30-06 because he thought the pain would go away. He sought medical treatment on 10-02-06 and he is restricted.

Employee was moving rocks in the muck pile to find the tooth of the shovel bucket. Employee had a rock in his hand and another rock rolled down on his hand smashing his left hand between the two rocks. Employee received a contusion to his left hand. Employee is to wear a splint but is doing his regular job.

Employee was building a pre-fab table to install in plant operator's control house. Employee was attaching upper cross member to the metal table when he caught his left hand on the corner of the table leg. He received a laceration that required 5 stitches. No lost or restricted time.

Employee was cleaning the yard. Employee reached down to pick up a piece of square tubing and did not know most of the tubing was stuck in the ground. When employee tried to pick up the tubing, he strained his right hand. Employee is restricted.

Employee and a co-woker were pushing screen deck into place as it cleared the back rail of the shaker. The screen deck dropped and pinched emloyee's left thumb between the back rail and screen deck. Employee received a laceration of his left thumb that required 3 stitches.

EMPLOYEE WAS PULLING ON A CONVEYOR BELT WITH ANOTHER EMPLOYEE. EMPLOYEE FELT A POP IN HIS NECK. HE THOUGHT HE WOULD BE OK. LATER HIS NECK BECAME STIFF & PAINFUL. EMPLOYEE WENT TO THE DOCTOR 10-20-03. HE WAS IMPROVING BUT STILL HAD SOME PROBLEMS. HE WENT TO THE CLINIC ON 11-10-03 AND WAS GIVEN PHYSICAL THERAPY TREATMENTS MAKING IT RECORDABLE.
