A miner got a piece of slag/metal stuck in left eye and it had to be removed by the eye doctor. 0n 4/8/2023. The miner was wearing a proper welding helmet and safety glasses while welding during the day on 4/7/2023.

Cleaning up materials on the decking around the new crusher platform. Employee had harness on, the lanyard caused employee to fall into the hopper opening, striking lower back against an I-beam.

While unplugging a chute, a piece of screen cloth had to be lifted & supported. The material pushed the screen cloth off, striking EE's ear.

He was removing a water valve from a pipe and bumped his head on pipe. He received a cut to scalp in back of head. Received staples and tetanus shot.

EE MISSED STEP WHEN EXITING YARD TRUCK. HIS LEFT FOOT LANDED ON A SMALL PIECE OF CONCRETE WITH ALL HIS WEIGHT. BROKE HIS FOOT.

GRINDING ON THE EDGE OF STEEL SHEET - GRINDER WHEEL CAUGHT ON A STEEL CLAMP - KICKED BACK STRIKING LEFT KNEE - PRODUCED A CUT THAT REQUIRED STITCHES.

WHILE BREAKING INTO FROZEN STONE PILE, SUFFERED NECK PAIN, WHEN HE JERKED BACK & FORWARD. SAID HE FELT SOMETHING SNAP IN NECK.

STUMBLED OVER WELDING CABLE & HIT ELBOW ON EDGE OF CHUTE. WENT TO DOCTOR 2/18 AND GIVEN PILLS ONLY. WENT TO DOCTOR 2/25 AND ELBOW WAS LANCED TO DRAIN OFF FLUID.

DOING MAINTENANCE WORK ON MOTOR BASE - SLIPPED OFF MOTOR BASE & SPRAINED WRIST.

GETTING MATERIAL FROM BIN. HIT SIDE OF BIN WITH A SLEDGE HAMMER, HANDLE CAUGHT ON AN AIR LINE CAUSING LEFT HAND TO SLIDE OFF THE HANDLE AND SLAM FINGER INTO AIR LINE.
