Cleaning off part of plant, did not use provided gloves, received large sliver.

Injured Employee set a piece of welding equipment up on the frame of a machine and was struck when EE ducked under the frame and the equipment fell down. EE's hard hat stopped most of the damage, but it was pushed off and EE received a large gash on the back of head. EE was taken in for treatment. EE missed two days and has been released for work.

Employee states he was lifting screens and pulling cords.

Employee reported that he was sweeping a sample box through product coming off conveyor belt to collect aggregate material when he felt a sharp pain in his right shoulder.

Employee was taking the counterweight cover out of the cone crusher. When he pulled back to get it loose, his hand hit a piece of brass bushing, and it peeled away some of the skin on his hand.

Removal of wheel cover plate from primary crusher for transport. Lowering wheel cover plate into feeder, struck left hand/thumb. Fracture to left thumb.

Employee Dropped a heavy metal shim on his left foot.

Employee states he was putting some lacing in the weigh bin when his left shoulder popped. He sought initial medical treatment on 8/29/11.

Employee was trying to close and latch dozer door frozen due to cold weather when he slammed the door on his finger.

Employee was working on cone change when he got a piece of steel in his eye.

Employee was throwing rocks out from behind a boulder; while he was throwing, he was turning his back. Reports back strain.

Employee reports pinching finger between screen box and conveyor while folding screener wing.

While changing cutting edges, bolt spun around and cut hand.

Employee was finishing up work on loader and putting tools away. Experienced sharp pain in middle back under shoulder blade. Employee states he was twisting and reaching in an awkward position.

Employee pinched finger while unhooking dollies from dozer.

Putting toggle seat in pitman on breaker. The seat slipped and pinched finger.

Employee states he felt a twang in his shoulder while pulling electrical cords.

Employee was changing roll plant screens. He scratched the tip of his elbow on the edge of the screen frame. An infection formed in his arm.

Employee was trying to pull the wheels out from the stacking conveyor. He was pushing quite hard on the wheels when he felt a "pop" in his right knee.

EMPLOYEE WAS CLIMBING DOWN LOADER STEPS. CAUGHT HIS RING FINGER ON LOADER STEP, CAUSING INJURY TO FINGER.

CONVEYER SET SCREW WAS HITTING GUARD. EE WAS POINTING OUT THE LOCATION TO ANOTHER CO-WORKER WHEN HIS GLOVED HAND WAS PULLED INTO THE MOVING MACHINERY. TIP OF HIS POINTER FINGER WAS PARTIALLY SEVERED.

EE WAS FINISHING HIS JOB ASSIGNMENT, PUTTING HISROLL WELDING MACHINE IN HIS TRUCK. EE REPORTED LIFTING EQUIPMENT INTO TRUCK AND HURTING HIS BACK. EE REPORTED MUSCLE STAIN.

EE WAS LOADING A PIECE OF STEEL INTO HIS TRUCK WHEN STEEL SLIPPED AND CUT HIS HAND.

EMPLOYEE WAS SHOVELING NEAR THE CRUSHING PLANT; HE LOOKED UP AND A ROCK FELL FROM THE SCREEN DECK AND HIT HIM IN THE FACE AND NOSE.
