Holding cutting edge bolt in place, impact wrench caused bolt to spin and cut index finger on employee.

Operator was using a cut-off wheel and disk came apart. Small section struck operator in the stomach, requiring 4 stitches to close the laceration.

The injured employee was blowing dust off the plant with compressed air, while blowing the dust off they broke protocol and inserted the air nozzle behind a guard on the under side of a tail pulley. The pulley yanked the nozzle from the employees hands cutting their palm.
The mine foreman was cutting a damaged piece of belting out with a utility knife, EE placed EE's hand in a bad position while cutting and slit EE's hand. EE drove to a local clinic where they gave EE 4 stitches. EE returned to work, total time away from work approx. 4 hours.
