Piece of metal got into employees' eye sometime during shift, unknown how it happened.

Holding cutting edge bolt in place, impact wrench caused bolt to spin and cut index finger on employee.

Operator was using a cut-off wheel and disk came apart. Small section struck operator in the stomach, requiring 4 stitches to close the laceration.

The injured employee was blowing dust off the plant with compressed air, while blowing the dust off they broke protocol and inserted the air nozzle behind a guard on the under side of a tail pulley. The pulley yanked the nozzle from the employees hands cutting their palm.

The mine foreman was cutting a damaged piece of belting out with a utility knife, EE placed EE's hand in a bad position while cutting and slit EE's hand. EE drove to a local clinic where they gave EE 4 stitches. EE returned to work, total time away from work approx. 4 hours.

Employee states they were stepping down from a ladder when they twisted their left foot/ankle.

Employee reported that he was sweeping a sample box through product coming off conveyor belt to collect aggregate material when he felt a sharp pain in his right shoulder.

Employee Dropped a heavy metal shim on his left foot.

Employee states he was helping to install a deck when he slipped and pulled a muscle in his right arm.

Employee was working on cone change when he got a piece of steel in his eye.

Employee was throwing rocks out from behind a boulder; while he was throwing, he was turning his back. Reports back strain.

Unknown at this time.

Employee was pulling on a pipe wrench to change spouts on fuel tank. Employee reports pain in chest area due to stretched ligament.

EE WAS WALKING DOWN LAB STEPS- TWISTED ANKLE WHILE STEPPING OFF STEPS ONTO UNEVEN GROUND.

EE WAS CHANGING THE CUTTING EDGE ON LOADER. BOLTHEAD CUT FINGER.

EMPLOYEE WAS EXITING JOB TRAILER WHEN HE SLIPPED AND FELL ON THE STEPS, CAUSING INJURY TO HIS LEFT KNEE.

EE WAS CUTTING A BELT LACING WITH A UTILITY KNIFE WHEN THE KNIFE SLIPPED AND CUT HIS LEG.
