While reinstalling hinged inspection door on main system fan housing, EE pulled awkwardly resulting in back pain. EE sought medical attention and was prescribed medication and given restrictions. Axiom was notified after the fact and is currently managing the case.

Showing all 32Employee lacerated the middle finger on right hand while installing a brash bushing into a journal. Employee was taken to the hospital and was treated and released without restrictions. The physician prescribed antibiotic medication.
Employee was closing hatch lid on control valves of Prill truck when it slipped out of his hand and dropped on left forearm which was underneath it laying down the support bar.
Dome light bulb in CAT 777B haul truck popped and got some glass in employee's eye. Glass was washed out but apparently resulted in chemical conjunctivitis for two days of restricted duty
While performing normal duties as a driller, the employee felt a gradual strain develop in his back
Employee was lifting 20lb lid to bucket elevator and felt discomfort in his lower back. Employee was seen by a physician who determined it to be a strain. Employee was given prescription medication and released back to full duty. The employee indicated he had been experiencing intermittent flares of lower back pain, including an episode two months ago after a baseball game.
Driller failed to remove deck bushing from upper sub per procedure and while trying to thread the 2nd section of steel the top of the carousel hit the safety wrench, breaking the chain which allowed the wrench to fall, glancing off the helper's hardhat the as he was visually checking to ensure that the steel was threading properly positioned outside of the cab on the drill deck.
Employee was welding wear iron onto the bottom of a detached loader bucket. He got help to lift steel onto tabs already in place to support steel, when one failed allowing it to fall to the ground catching his glove on the way down trapping his arm underneath. Resulted in a significant bruise to left forearm requiring anti-inflammatory prescription and follow-up physical therapy.
Third party worker was transferring bags of barite material to a new pallet because there was a busted bag at the bottom of a stacked pallet. Worker felt discomfort in his right shoulder blade. Worker was sent to medical by his agencies company services.
Employee went to doctor after he was terminated for cause and claimed he hurt his shoulder while shoveling and lifting at work. (Questionable claim).
Maintenance employee caught right thumb between rotary slide plate and exterior portion of mill bin, causing crushing injury to right thumb tip.
While climbing up to the cab of a track hoe the employee felt a sharp pain in his left calf. He said he didn't slip or anything else; he just felt a pull in the muscle.
Maintenance employee was changing out drive belts on mill motor, while replacing belts employee's finger got pinched between belt and pulley causing minor laceration and partial fingernail dislodgement. Employee was transported to Occ Medical Clinic for treatment.
Taking down a gear box from a conveyor belt. The employee states that the gear box "slid" and made contact with his finger causing the fracture.
The auger on the ANFO truck was stuck. EE removed the inspection plate cover from the vertical auger and put his hand inside to remove the blockage. The auger moved pinching his fingers between the auger and the auger housing.
Employee cleaning off table in maint shop, got object in left eye. Employee tried to remove object. Employee called foreman after his shift and wanted to get treatment. Foreman met employee at hospital, where Dr removed piece of metal. Employee released for work. Employee went to hospital the following day for follow up. Dr found rust in eye. Employee sent to eye Dr., released.
Employee was working on a forklift repairing the main lift cylinder, due to a mechanical failure. The clamp and bracket holding the cylinder in place had come loose. The employee was loosening the clamp cylinder. The forklift was not properly secure from movement while loosening the clamp; the mast dropped about 6 inches striking the employee's right shoulder.
While changing teeth on loader, pin punch broke & fragment struck EE on chin. Cut needed 1 stitch no impairment.
EMPLOYEE ALLEGED WHILE INSTALLING DRIVE BELTS ON MILL MOTOR HE PULLED ON THE BELT AND HIS FINGER WAS PINCHED BETWEEN THE BELT AND THE PULLEY.
Crusher operator was trying to redirect a very large rock into the jaw crusher, when it fell the wrong way and contacted his left toe. The rock impacted his steel toe boot into his left big toe
Employee was unplugging cone crusher when he felt a pain in his right hand between his thumb and first finger. Cold pak was applied on site, employee finished working his shift. After employee had returned home swelling returned and he went to the emergency room for evaluation. Was placed on light duty.
WHEN EE WAS INSTALLING JIG SCREEN INTO #1 CELL OF #2 JIG, THE SCREEN STUCK HE FORCED SCREEN AND IT BROKE FREE CAUSING HIM TO LOSE CONTROL OF SCREEN AND HAND POSITION CATCHING TIP OF FINGERS BE TWEEN JIG SCREEN AND SCREEN FRAME.
EE WAS FUELING BLADE, HE WAS CLIMBING BACK UP TO FUEL TANK WHEN HE BUMPED HOSE CAUSING IT TO COME OUT OF THE TANK AND HIT HIM IN THE CHEEK, CRACKING A TOOTH.
EE WAS STACKING MATERIAL ON PALLET AND FELT PAIN IN LEG AND BACK.
EE WAS UNABLE TO DETERMINE THE ACTUAL TIME AND ACTIVITY THAT RESULTED IN THE BACK PAIN HE REPORTED. THERE WAS NO PAIN AT START OF SHIFT BUT STARTED JUST BEFORE HE REPORTED IT. THE PAIN WAS MOD ERATE AT REST BUT WORSE WHEN BENDING OR TWISTING.
A ROCK THAT HAD BEEN LIFTED UP OUT OF THE JAW CRUSHER ONTO A DECK ROLLED BACK ONTO EE'S FOOT.
EE ALLEGED THAT WHILE STACKING BAGS ON PALLET HESPRAINED HIS BACK WHILE LIFTING A BAG.
EMPLOYEE ALLEGED THAT IN THE PROCESS OF LOADING GEL INTO BULK BAGS AND REMOVING THE BAGS FROM THE BULK SCALE HIS FOOT SLIPPED OFF THE SCALE AND SCRAPED HIS LEFT CALF ON PART OF THE METAL SCALE .
EE PINCHED RIGHT FOURTH FINGER IN MILL ROLLER DURING ROUTINE MAINTENANCE. XRAY REVEALED NO BROKEN BONES.
EMPLOYEE ALLEGED THAT WHILE PUTTING TRANSMISISON OIL IN THE PAYLOADER. HE WAS BENDING OVER ATTEMPTING TO LAY 55 GAL DRUM ON ITS SIDE (SO IT CAN BE PICKED UP WITH CABLES) HE FELT PAIN IN HIS LE FT LEG.
EE WAS CHANGING PIPE FOR AIR LINE TO ROTARY VALVE. GOT FINGER CXAUGHT IN VALVE. CUT OFF TIP OF FINGER. (BACK TO 1ST KNUCKLE).
DURING THE PROCESS OF ATTEMPTING TO LOCK BULKHEAD DOOR, EE SMASHED RIGHT HAND THUMB BETWEEN BULKHEAD DOOR FRAME AND LOCKING HANDLE.
