In the process of building a brattice; the ee was placing a block when another ee tossed him another block-catching the injured ee's finger between the blocks, resulting in a fracture to the tip of the finger. There was no medical attention administered, only diagnostic and first aid.

EE had finished installing roof bolts and was advancing the line curtain to provide ventilation to the face. He raised himself up off his knees and felt a tightness in his left knee. EE was administered first aid and transported to the surface where he was taken to the local hospital.

EE had finished a cut and was backing the miner out of the face. EE began to install curtain and was reaching to put up the curtain and he felt a "pop" in his right knee. He was transported to the local hospital.

EE was pulling cable on a roof bolter when he felt a "pop" in his left knee. He continued to work but the knee "popped" again. The ee later reported pain in his knee & went to the local hospital for exam. He was referred to a panel dr. for a follow-up.

An electrician was working on an airlock door located near the portal of the mine. The electrician was dismantling the guard for the roll-up garage type door used as the outby airlock door. The guard suddenly moved upward pinching 2 fingers on his right hand resulting in lacerations to his middle and index fingers requiring 3 sutures to close the lacerations.

A loaded shuttle car damaged the ventilation curtain leaving the #3 heading. The ee repaired the curtain and placed a rock on the bottom of the curtain to hold the curtain in place. The ee received a laceration to his left hand from the sharp edges of the draw rock - requiring four sutures.

A third shift utility crew was dismantling a conveyor belt take-up unit. The ee attempted to block up the unit to allow a scoop bucket to be positioned under the unit. The take-up inadvertently slid off the scoop bucket resulting in the ee's right hand being caught-between a metal block and a piece of angle iron located on the mine floor.

The ee was attempting to remove a rock from the chute of the #1 belt dump over point that had gobbed off. As the ee moved the rock - a piece of metal band that could not be seen by the ee cut through his gloved hand resulting in a laceration to his ring finger on his right hand. The ee received 5 sutures to close the laceration.

The contract employee entered the mine at 9:00 a.m. He stated to two co-workers that he had injured his back over the weekend. At 1:05 p.m. he called to the mine surface and alleged to the mine superintendent that he had injured his back at the #5 belt drive. Abby Contractors is controverting the workers compensation claim.

The ee alleges an injury to his back while dismounting a personnel carrier. On August 18th the ee denied any event that caused injury. The ee left the mine and mine property on his own without medical assistance.

The employee was helping the continuous miner operator move the machine. After pulling the miner cable pull rope and hooking the rope to the miner, he began having lower back pains. The employee finished his shift, but went to the doctor the following day due to the pain becoming worse. The ee was placed on restricted work duty.

Two employees were installing leaky feeder cable. After the cable was installed the two employees were attempting to remove the cable spool from the cart when the reel moved and caught the ee's middle finger on her right hand between the reel and the cart frame resulting in a laceration to the tip of her finger.

The steel cable for the B-2 belt drive take-up unit had traveled outside of the wheel guides. The injured employee was attempting to return the cable into the guides when the trapped cable became free and his left hand was caught between the steel cable and the wheel. This resulted in a laceration to the employee's index finger on his left hand.

The section crew was in process of pulling back the section power center and the employee was helping configure the high voltage cable as the cable was being pulled by a scoop. The employee's left leg was caught under the cable, resulting in the employee's left ankle being injured.

The employee was working as a belt drive person performing routine clean-up around the #6 drive area. The employee moved a metal plate (24" X 36") from the drive area and was in the process of positioning the plate against the coal rib when his little finger on his right hand was caught-between the metal plate and the rib.

Employee had drilled a 10' hole to install a cable bolt. As he was bringing the drill steel out of the hole; the pusher steel and the first extension stuck together. As he was pushing & pulling the steel to pull it apart; the steel came apart and one piece hit him in the mouth chipping two teeth.

After replacing belt in a belt line, the ee was using a chain hoist to pull the belt together to splice. The crew had loosened a top roller and the ee placed his arm at that roller while operating the chain hoist handle. The chain caught the roller and raised it up. The forearm of the ee was caught between the rigid belt structure & the roller.

The employee was helping another employe put a compartment cover back on the roof bolting machine. The cover had been removed to wash the compartment out. The employees lifted the cover back on to the roof bolter and were sliding the cover in place when the employee caught his finger between the cover and the frame of the machine.

Employee was assisting with the loading of a 1000 ft. piece of conveyor belt. The roll was on a stand/ the steel band holding the belt had been cut. Employee was pulling the steel band from the belt & the end of the steel band came out from under the belt & sprang back striking the employee on the neck causing a 2 inch laceration which required 7 stitches to close.

While performing a belt conveyor move the employee was attempting to route a flexible water line under the conveyor when his rescuer became hung on the belt structure of the idled belt conveyor. While attempting to free the rescuer the employee felt a pain in his back.

The EE was bolting the No. 2 left crosscut and had drilled the last roof bolt hole in the place. The coal height was too low in the area to put the 42" resin roof bolt in the hole. As he was bending the roof bolt slightly to get the bolt in the hole, he felt a pain in the right side of his lower back, near his rib cage.

THE TIMBER MEASURED APPROX. 56" LONG X 4" SQUARE.
