Showing all 7Mechanic was using 2 lb hammer to drive pin into valve while performing maintenance activities. As the hammer struck the pin, the employees 5th finger (pinky) struck metal valve mashing finger between hammer handle and valve. The employee received 7 stitches in 5th finger. Restricted duty only, no lost days.
At 8:45 am on 7/16/2012, a maintenance technician received a laceration to the right thumb requiring 5 stitches to close. The technician was attempting to remove the insulation on an electrical lug nut in preparation for changing out a centrifuge motor. As the technician was cutting the insulation, the knife slipped, making a one inch cut on his thumb.
Employee was cutting and removing straps from a container he was preparing to load. During the process of cutting a strap, the knife made the cut and continued towards the employee's face, striking him below his safety glasses and right eye.
Employee was working with a shovel in a tank when underside of left forearm contacted blade of agitator. Agitator's blade was sharpened on edge, causing laceration to forearm.
EE was in the shop working on a spray bar. He was installing a spray nozzle(approx. 3/4" x 11/2" long) into a 2" pipe. The wrench he was using to thread the nozzle into the pipe slipped off, causing him to twist his back. At the emergency room, he received an injection for the pain.
EE was cutting the end of a Robroy condulet to remove excess in order to insert plug. He had his left hand stabilizing the pipe, about one foot up from the end. He was cuttig the end of the condulet with a razor knife in his right hand and pulling up. The blade slipped and cut his left thumb, resulting in two stitches.
