Showing 50 of 488Employee was using an 8-foot steel bar at the P 9/10 chute to remove a slab (rock) at the bottom of the chute. As the employee was attempting to hit the slab, their left hand hit the bottom of the chute door and smashed their left pinky finger.
Morenci Mill Maintenance employee was in the belt shop attempting to cut some skirting for a belt job. As they began to cut using the Olfa knife, the knife slipped cutting into the A6 rated glove and HexArmor sleeve into the employees left thumb, side cutting the skin next to the nail.
An operator was using a rock hook to unplug a chute. They were pulling up on a stuck rock when it broke loose which caused the tool to come up and make contact with the employee's central incisor #8 (front upper) tooth.
Employee was called to assist in cleanup of a floc spill; Employee was using a shovel to clear the floc, slipped and fell, hit the right side of their face on the concrete, causing a laceration in right eyebrow, required sutures.
Employee was stripping/cutting the exterior jacket of the 25kv cable when the insulation removal tool (knife) slipped and cut across the top the left index finger.

Medical provided performed fingernail trepanation & issued oral oxycodone & Ibuprofen. Second opinion requested -Stated that with conservative care such as O/C ibuprofen /ice packs would achieve the same desired outcome as FAI.

While installing banjo bracket and arm on the shovel sticks, using a come along, the come along chain failed and the bracket and arm contacted the employee's right wrist area. EE was taken to the ER, diagnosed with a fracture and taken off work. CURRENTLY ONGOING
A mechanic was working on a mast raise cylinder. Attempting to loosen a flange bolt with a wrench and it would not loosen. The mechanic then grabbed a 2lb hammer and struck the wrench several times. On the last strike they missed the wrench and struck their left hand in between the finger and the thumb

EE was working on cleaning out mixer. EE picked up a pry bar to break loose hardened material from mixer flights. While doing so, the ee hit the buildup of material and the force from pry bar hit EE in the face near EE's eyebrow / forehead area. EE suffered a laceration and needed stitches.

Employee was dressing lead anodes for cell maintenance. While installing side insulator (PVC) with a rubber mallet, the left side of the anode slipped off the strongback hook, striking the employee in the neck area. EE was taken to the ER and taken off work 2 days for concussion protocol.
Changing tire on skid steer, employee was using a ratchet strap to wrap tire to aid in seating the bead for inflation. Once the bead was set and able to receive air the rachet strap was released and the handle sprung back and caught the employee in the right forearm causing a 2" laceration that required 5 stiches.
Employee was preparing to remove a tire on an articulating truck, and when placing the bead breaker in position, caught the tip of index finger between the bead breaker and the rim of the tire. Lacerated finger, requiring sutures to close.

At approximately 7:05 AM, a crew was transferring buckets of concrete cure and seal to a new pallet due to damage on the original. One bucket had leaked and adhered to its cardboard base. After failed removal attempts, a utility knife was used to cut around it. During the cut, the blade slipped and struck a worker's leg above the knee.

A Blasting Operations Technician was opening a bag with a utility knife and cut their leg. This caused a laceration that required stitches. The employee returned to work the next shift.
Employee was braiding a piece of rope. They cut the end of the rope and cut their ring finger.
Employee was working on a buggy whip when the screwdriver being utilized slipped and penetrated the right index finger. Employee received stitches and was released to work.

EE was conducting work in the mill underflow tubs by chipping away steel ball chips from the grating when a piece of metal went into their eye and began causing discomfort. EE was taken to get medical treatment and was prescribed an antibiotic.

Employee was working on grader replacing cutting edges. The employee over torqued a bolt causing it to break loose. The bolt hit the underside of employee's hardhat ricochet to employee's forehead striking employee above the right eyebrow, creating a laceration. The laceration needed 5 stitches to close.

Employee was pressing on cell line when bar fell in and splashed liquid in eyes. EE was taken to the ER, prescribed multiple application antibiotic, referred to ophthalmologist, who in turn took EE off work for next shift.
Employee was loosening a nut with a wrench when the wrench slipped. The employee lost their footing and fell, the employee used their hand/arm to catch self and injured left wrist
EE was in the process of changing a pin on the incher. When EE tried to remove it by hitting it with a hammer, a piece of the hammer metal came off of the pin and went into EE's leg. Embedded piece was not removed.
Employee was trimming overflow rubber off edge of conveyor belt when knife slipped and contacted the employees knee resulting in sutures. The employee returned to work on EE's next scheduled shift but is on restricted duty for 10 days.

Mine Maintenance Employee sustained hand laceration through leather gloves while assisting with maintenance on Dozer D10-24.
An employee was assisting another employee w/ removing a zip tie off a fuel can nozzle. EE was holding the can while a second employee was cutting the tie w/ a knife. Knife cut thru the tie & the momentum continued forward causing a laceration to the top side of the EE's thumb & index finger on the webbing. EE had stitches & issued restrictions.

The technician was using a tool to hold pressure up on a plow bolt to prevent the bolt from spinning while removing the nut. The tool slipped and the technician's hand contacted the spinning bolt head causing a laceration.
While trying to remove an inner tire on a forklift, employee was swinging a sledgehammer and struck the end of right index finger, lacerating & fracturing it. Employee received 4 stitches and given restricted work duties. Currently ONGOING.
