Maintenance Technician (M1) was tasked with installing the track on a Bulldozer. M1 used a hammer to align the track master link. During master link alignment M1 felt flying debris enter their right eye. M1 was transported to the onsite clinic for evaluation and then transported to an offsite medical facility.

EE reports via phone that while working to replace the dump cables on the #3 dragline, when beating them out of their sockets and missed the swing. EE reports that this missed swing caused the movement to snatch the EE left's shoulder. EE reports left shoulder pain with movement.

EE reports pulling on a wrench and experiencing a pop in the right-side chest muscle. EE states pain with movement of arm. On physical exam there is a tender lump in the muscle.

Maintenance tech. was in the process of replacing an air cannon @ the #2 gate east side. M1 had a ratchet in left hand and a 15/16 wrench in their right. The ratchet slipped off the bolt and struck M1 on the bottom lip. M1 immediately reported to the supervisor & went to the first aid clinic. M1 was carried to an offsite medical facility and received three dissolving sutures.

Employee (EE) reported to the Clinic on 03/12/2024 and stated that while attempting to loosen a bolt with a ratchet, while pulling handle towards their body, they felt lower back pain. EE was referred to Company Care. Restrictions were given that cannot be accommodated. EE is off work until further notice.

Operator 1 (O1) was tasked with placing stakes in the ground to identify dig limits near the dragline. O1 was using a steel hammer to drive stakes. O1 was holding the stake with their left hand while holding the hammer in their right hand. O1 struck their left index finger with the hammer which resulted in a laceration. O1 was transported to an offsite clinic for evaluation.

On 12/29/23 Employee was using a pry bar to open a door on a pit pump. The pry bar slipped and struck the EE in the face causing a partial thickness facial laceration. EE was treated at the onsite clinic and returned to work, with no restrictions or lost time to report.

On 09/27/2023 Employee-EE stated they pinched their right hand between a wrench and a cylinder and reported swelling and pain in the area. EE was referred to Co. Care for eval. and treatment. EE has returned to work with restrictions until further notice.

EE was using a pry bar to turn the pump assembly for alignment when the bar slipped and struck the employee on the nose. The impact caused a laceration to the left side of the nose. EE went to a clinic for evaluation and treatment.

EE was using a pry bar to take swing brake off swing motor. When EE applied pressure to bar EE experienced pain under right pectoral muscle, initially filed as report only. Employee contacted Clinic 12/15/22 requesting evaluation & treatment due to on-going pain stating unable to breathe normally or sleep normally due to pain. EE went to LCMC-ED for treatment. No restrictions.

Employee was correcting a vibration in a screener unit. Tech noted the issue was a loose bolt on the shaker box cylinder rod end. While attempting to pry the the cylinder enough to allow the R&R of the bolt, the bar slipped and pinched the employee's left index finger, lacerating the tip

EE was installing a packing in WBC matrix pump. EE was turning shaft to tighten impeller against the packing sleeve, using a 48" pipe wrench to turn impeller shaft. Shaft became difficult to turn but was not fully seated against packing. When EE pulled with force, EE felt pain in right elbow. EE was sent to company care and will be out of work, follow up appointment scheduled.

Employees were tying in a pipe on #4 Dragline area attempting to get bolts in pipe. Co-worker was using 1 5/8 wrench to tap the bolt in when injured employee placed a hand in the way causing wrench to contact left hand. EE transferred to Medical, appt scheduled with Ortho institute on 8/9/22. Possible crush injury to left hand.

Employee was tasked with removing screening panels held in place by wedges. While trying to remove the wedges with a hammer, the employee's hammer missed the intended target and struck employee's left thumb.

Operator (O1) noticed conveyor rubbing against structure. O1 determined contact was caused by material build-up near bend pulley. O1 used hammer to remove material build-up. O1's hammer made contact with the pulley, resulting in both the hammer and O1's hand being pulled toward the pulley. Fractured and lacerated left ring fingertip.

Employee was using a pry bar to remove a stump that was stuck in a pump when EE's finger became pinched between pry bar and a solid metal surface. Right ring finger has small bruise under the fingernail with slight abnormal bend to fingertip. EE was sent off-site for evaluation, x-ray was negative. Diagnosis was crush injury to tip of right ring finger, placed on modified duty.

Employee was tasked with replacing belts on a pump. As EE was turning the pump sheave with a pipe wrench, the momentum of sheave caused the employee's finger to get pinched between the pipe wrench handle & a belt guard mounting bracket. EE was seen by onsite EMT & sent to LCMC where x-rays showed a fracture in tip of finger. EE will have surgery on 6/23/21.

EE was clearing debris from hydraulic ditch with potato rake. EE was pulling up on rake, it got hung up and EE felt shoulder pop, felt a sharp pain and cannot move it past a certain point. EE went to an emergency room, referred to an Ortho clinic on 3/18. EE has not returned to work.

HEO was assisting a Ut.Op remove stump from suction of #4 pit pump. A 10' rod used to force debris from end of vertical section of pipe. They placed rod in suction inspection door & on stump, rod slipped off stump & dropped. Finger was pinched between flange of inspection door & handle of rod. HEO was taken to clinic then to hospital, staff cleaned wound & prescribed antibiotics.

Technician was removing an idler pulley from a bulldozer and was striking the pulley with a sledge hammer. A shard of metal was ejected and it struck the technician penetrating the technician's leg. The metal was imbedded in the leg and had to be removed by a medical professional.

Employee rigged a 90 degree elbow of a piece of pipe to be removed using a come-a-long. The employee affixed the rigging device by wrapping the chain and hook around the pipe. While trying to remove the pipe, the pipe shifted, and pinched the employee's finger between the pipe and the chain. Employee sustained a fracture and laceration to little finger of right hand.

Pipe crew had been tasked with removing a pressure sensor from the discharge spool of #18 ore line. Employee had been tasked with using box wrench to prevent movement by a nut located on the bottom of flange. Wrench slipped off of the nut it was wedged again and struck the adjacent pipe. At time of impact right ring finger was positioned between the wrench and pipe.
