Showing all 35Employee was installing a bearing on the left front strut on end-dump. Employee was hammering a bearing in place with a 10 pound hammer while holding it with his left hand. Employee's hammer become entangled with some hoses and missed the bearing and struck his hand with the hammer.
Employee was taking drain plug out of unit 1115 diff. Volvo 40F articulating truck. Hammer glanced off of breaker bar- pinching right hand index finger between hammer handle and breaker bar. Five stitches in index finger.
Employee was taking bearing races off a pump shaft when it he hit the race with a hammer the shaft slipped and fell and caught his little finger on his left hand between the end of the shaft and the edge of the bench he was working on. Cutting his little finger and requiring 11 stitches.
Employee was removing wheel studs from a truck wheel he hit his left hand below the thumb. A bandage was applied but it kept bleeding so he went to doctor and received 4 stitches.
EE was installing a cleaning lip on a loader. EE hit the lip with hammer causing a metal fragment to enter his right arm. EE went to E.R. EE went to another doctor and had the object removed on 8/28/13 and missed one day of work.
Employee was hitching water pump (1601) to IT loader (1045). He was cranking the jack up to align the hitch when the jack broke off the pump. The tongue of the pump fell on his right foot. His foot was bruised above his steel toe.
Employee was installing a pin into a hitch on a scraper unit No. 1122. Employee was hitting a pin with a sledge hammer. A piece of the pin broke off and hit him in the left thigh. (Had metal removed and 9 stitches).
Employee was working on a overflow box and trying to get it to set into place. He was using a chain come-a-long. The come-a-long came loose and the chain hit employee in the chest. (Contusion to the ribs) Employee went to E/R that night. Then worked two days and went back to doctor.
Employee was working on unit #1144 end dump. He was working in the cab while taking apart the fan box. his tool slipped in the bedding and the sharp point went in to his left hand requiring stitches.
EE said he hurt his back while fastening the compactor down on the lowboy for transportation to a different location.
employee was changing the oil in water pump #1578 the wrench slipped off the oil pan bolt and his arm hit a piece of metal on the frame. this caused a laceration on his arm. seven stitches.
EE was working on an engine on unit 1322 lube truck when and was pulling on wrench and his right shoulder popped. EE didn't go to doctor till the next day said that it hurt more the next day. Sprained right shoulder. EE worked on the 1/6/11 and 1/7/11.
Employee was repairing Komatsu dozer and was using an 1/2 impact wrench when a wobble sock on the wrench came off and hit him in the mouth. Contusion and broken tooth. He was removing bolts from 475 Komatsu dozer drive shaft.
EE was hitting the end of a carrier roller shaft that had worked out of the trackframe of the dozer. When he hit it the second time a piece of metal chipped off of the roller and pierced his leg around the back of the knee. He got 4 stitches.
Employee was working on #2 belt press. He was using a ratchet chain fall to move a roller back. His hand slipped off the chain fall handle hitting his right hand on the belt press cutting his hand requiring 2 stitches.
EE was using a sledge hammer to chip frozen material from a mud bin. EE stated that he had lower back pain at the end of his shift.
Employee was digging tracks on shovel and a rock fell off of the track frame and landed on his foot above the steel toe fracturing his left foot.
Employee was holding a punchas it was being hit with a hammer a piece of the punch broke off and hit him on the left side of his face the piece of metal was embedded in his cheek. It was removed and he received three stitches.
Employee was installing bushings in shovel bucket. Bushing was crooked & stuck, employee was hitting the bushing with sledge hammer, a piece broke off the bushing & hit him in the stomach entering the muscle tissue. Went to medical doctor on Oct. 25. Released. Went back on Oct. 28. Metal was removed. 2 stitches.
Employee was helping change a course coal basket in the washplant. Employee was using a sledge hammer hitting on the course coal basket puller. His left shoulder was hurting and a knot came up on the back of his left shoulder. Employee went to doctor on 08-15-2007.
Employee was working on the r.c. screen in the washplant, trying to loosen a bolt with a wrench. The wrench slipped off the bolt causing his left index finger to be cut. He received four sutures.
Employee was working on the undercarriage of unit 1230 dozer. He had heated a pin with a torch & he hit the pin with a hammer. A piece of metal got into his eye, he had a corneal abrasion.
EMPLOYEE THOUGHT HE MIGHT HAVE STRAINED HIS SHOULDER CLEANING TRACKS ON DOZERS. HE COULDN'T REMEMBER AN INCIDENT WHERE HE HURT IT. JUST KEPT GETTING WORSE & HE WENT TO THE DR. ON 1/17/05.
EE was attempting to break a large piece of coal that had blocked the feeder chute that feeds the wash plant. He climbed onto the belt. When the coal broke loose he fell on the belt on his right knee. Did not report as injured.
EE was working in the wash plant assisting other workers with the removal of a steel coal chute. The chute was being lowered down with a chain hoist and became lodged on the flange support beam. The main load bearing chain became unhooked and allowed chute to fall to floor. Miner was struck on right leg/ knee and strained lower back.
EE WAS DIGGING THE TRACKS OUT ON UNIT #1011 SHOVEL. A PIECE OF MUD FELL OFF THE FRAME OF THE TRACK AND HIT THE FINAL DRIVE AND HIT EE BEHIND THE KNEE.
EE WAS TRIMMING A PIECE OF WIRE USING A RAZOR KNIFE. THE KNIFE SLIPPED AND LACERATED EE'S LEFT INDEX FINGER. REQUIRED 5 SUTURES TO CLOSE. EE WAS TREATED AND RETURNED BACK TO WORK.
ON 4/13/04, EE WAS CUTTING A METAL BAND FROM A BUNDLE OF ROOF BOARDS WHEN THE METAL BAND BROKE & STRUCK HIM ON THE RIGHT HAND CAUSING A LACERATION. HE WAS EXAMINED & TREATED BY A LOCAL PHYSICIAN WHO CLOSED THE WOUND WITH SIX (6) SUTURES & RETURNED HIM DIRECTLY BACK TO WORK.
EE WAS ATTEMPTING TO CHANGE THE HEAD SOCKET FROM A ROOF BOLT WRENCH & AS HE STRUCK IT WITH A HAMMER HE ACCIDENTALLY CAUGHT THE END OF HIS LT.LITTLE FINGER BETWEEN THE EDGE OF HEAD SOCKET & THE HAMMER, CAUSING A FRACTURE AND A LACERATION TO HIS FINGER. HE RECEIVED SEVERAL SUTURES TO CLOSE THE WOUND, HIS FINGER WAS FITTED W/PROTECTOR & PROPERLY WRAPPED TO KEEP CLEAN, RELEASED TO WORK
EMPLOYEE WAS INSTALLING MINER BITS ON A CONTINUOUS MINING MACHINE AND AS HE STRUCK A BIT WITH A HAMMER TO SET THE BIT A PIECE OF METAL BROKE AWAY FROM THE BIT AND STRUCK HIS RIGHT INDEX FINGER CAUSING A FRAGMENT OF METAL TO BECOME LODGED IN HIS FINGER. THE METAL FRAGMENT WAS REMOVED VIA A SMALL INCISION AND CLOSED WITH TWO SUTURES.
EE WAS USING A HITCH PIN TO CLOSE THE EARS ON A 4" CAM LOCK FITTING WHEN INSTEAD OF HITTING THE FITTING HE STRUCK HIS LEFT HAND WITH HIS RIGHT HAND HOLDING THE PIN.
EE WAS PUTTING A NEW SET OF TEETH ON THE HITACHI SHOVEL, HE WAS DOING SOME TRIMMING ON THE BUCKET WITH A TORCH, HE WAS USING A HAMMER HITTING THE METAL. A PIECE CHIPPED OFF AND WENT INTO HIS L EG. HAD SOME NUMBNESS IN LOWER LEG.
EE WAS TRYING TO CUT A RUBBER HOSE WITH A POCKET KNIFE. THE KNIFE SLIPPED OFF THE HOSE & CUT HIS LEFT THUMB, REQUIRING 5 STITCHES.
EE WAS USING A 4 LB. HAMMER TO DRIVE NAILS INTO A BELT SPLICE WHEN THE HAMMER GLANCED OFF A BELT COVER CAUSING HIM TO STRIKE HIS LEFT HAND WITH THE HAMMER.
