The individual was using a chain ratchet to work on a tail piece. The hook slipped hitting him in the forehead. The individual received stitches and saw doctor on following day. (10-1-09). This is medical only.

EE was plastering on overcast wall outby section of Rt. Mains. EE had to adjust safety glasses and slid them down on the nose to un-fog. EE was putting plaster on underside when the B-bond plaster splattered into the employee's eyes. The employee was brought to the surface, eyes flushed out. Taken to the hospital, eyes examined, Lt. eye scratched, dialated eyes & medication.

EE was cleaning out material from under chain lid on coal feeder. While replacing the lid, the slate bar slipped, causing the lid to slide and mashed and cut the EE's right hand between the lid and the coal feeder. This resulted in having 4 stitches to the top of the EE's right hand.

Was drilling a hole for a roof bolt when steel stopped up. He hit steel with a drill wrench trying to unstop it. Wrench ricocheted back striking him in the right cheek.

The employee was changing the operator's side feed cylinder on a roof bolting machine. A come-a-long hoist was hooked to the machine's boom and a roof support plate. The hook came loose from the roof support plate and the boom fell catching his arm.

The floor jack contacted his right little toe, causing a fracture and laceration. He was positioned on his knees at the time of the injury.
