The employee was helping make a belt splice on the #1 belt, when the belt splicing tool got jammed. As the employee was helping to free up the belt splicing tool, his left hand got caught, causing a fracture to his left thumb.
Employee was helping remove a bad pump motor on the miner, the chain hoist that was being used to remove the bad pump motor broke loose and hit the employee on the left knee.
Employee was using a razor knife to open a rock dust bag when employee cut his left finger. The laceration required 3 sutures.

INDIVIDUAL WAS IN KNEELINE POSITION ALONG BRLT PREPARING TO SQUARE IT FOR A SPLICE. AS HE PULLED HIS KNIFE THROUGH THE BELT, IT CAME OUT AND HE STABBED HIS LEG.
INDIVIDUAL WAS CUTTING CONVEYOR BELT TO SQUARE IT USING A UTILITY KNIFE. AS HE PULLED THE KNIFE THROUGH THE BELT IT CAME TO A THIN EDGE CAUSING HIM TO PULL THE KNIFE ON THROUGH INTO HIS KNEE.
EMPLOYEE WAS JACKING THE BELT WITH A COME-A-LONG. SUBMITTED NOW AS A RESULT OF AN AUDIT.
