Showing all 33Crew were taking a belt drive apart. The boom was being held up with come-along while pins were removed. Chain broke letting boom fall down against drive frame catching employee's hand.
Employee was attempting to adjust spotlight on off-loader barge by tossing a rope around the light bracket. During the tossing motion, employee pulled muscles in his back. Employee didn't inform mgmt. that he missed work on 6/25, due to this incident, until 7/3/07.
Tightening wheel on truck with lug wrench, strained abdominal muscle.
Employee was supervising removal of slide plate. Was attempting to hook 2nd come-a-long to "I" Beam when hooked come-a-long slid from "I" Beam falling approx. 4' hitting employee on right hand pointer finger causing possible fracture of finger.
Was lifting car tounge up to move into position to couple up. Pain middle back.
EE was installing a new run of conduit. During the conduit run, a bracket from the old conduit needed to be removed. The anchor bolts from the old bracket were loosened & EE tried to pry the bracket the rest of the way out of the wall. At this time, the bracket fell and grazed EE's left index finger, resulting in a laceration requiring 3 stitches.
EE was tightening an anchor bolt on a counterweight guide beam. The crescent wrench he was using slipped off of the anchor bolt, causing EE to hit an adjacent anchor with his right fifth digit finger. This resulted in a bruise to the finger and EE was given 3 days off by his attending physician, therefore resulting in a lost time accident.
Was making splice miner cable that had shielding. Pulled on shielding to cut. Utility knife slipped. Cut left thumb and web of left hand.
Employee was working on a roof bolter, screwdriver fell off boom and struck right eye.
Employee recieved small cut right hand while opening an oil can.

Employee was splicing a water hose with a banding tool. When he was finished, he pulled out what was left of the band. During the removal the extra band that was left in the banding tool, cut the left forearm of the employee.
Employee was striking at a timber with a pick hammer and missed the timber striking himself in the right thigh. Started missing work on 6/29/2004.
Employee was making a cable splice and while hitting the knocker a piece of metal chiped off hitting him in the right thumb. Required 4 stitches.
Employee was splicing a miner cable and while striking the anvil a small piece of the hammer chipped of striking him in the left thumb. Required 3 stitches.
WAS USING SLATE BAR TO MOVE HYD HOSES OUT WAY TO INSTALL DRIVE SHAFT ON SCOOP. BAR SLIPPED MASED RIGHT RING FINGER.
EE WAS CUTTING THE SUCTION HOSE ON THE ROOF DRILL AND THE KNIFE SLIPPED CUTTING HIS FOREFINGER OF HIS LEFT HAND, REQUIRING SUTURES. EE RETURNED TO WORK AT NORMAL DUTY ON HIS NEXT SHIFT.
EE WAS WORKING ON A JOY SHUTTLE CAR WHEN THE CHAIN ON COME-A-LONG BROKE STRIKING EMPLOYEE'S LEFT HAND.
REPAIRMAN MAKING CABLE SPLICE, CUTTING CABLE OPEN, KNIFE CUT THROUGH CABLE AND CUT UPPER RIGHT THIGH.
EE WAS USING A HAMMER TO DRIVE IN A WEDGE OVER THE COVER ON A DRAIN AND RINSE SCREEN. AS HE WAS MAKING A STRIKE, THE HAMMER STRUCK AN OVERHEAD BEAM, CAUSING THE HAMMER TO CHANGE DIRECTION AND STRIKE HIS LEFT INDEX FINGER. SUTURES WERE REQUIRED TO CLOSE THE WOUND.
EE WAS DRIVING A BEARING INTO A DOGBONE ON 9222011 AND A PIECE CHIPPED OFF HIS SLEDGEHAMMER AND WENT INTO HIS FINGER.
EMPLOYEE WAS USING A HAMMER TO KNOCK A BEARING OFF A DRIVE SHAFT WHEN A PIECE OF METAL FROM THE BEARING BROKE OFF AND PENETRATED HIS LEFT HAND. THE PIECE OF METAL WAS SO SMALL THAT IT POSED NO PROBLEM (ACCORDING TO DOCTOR) BUT DOCTOR ATTEMPTED TO REMOVE IT. COULD NOT FIND METAL BUT INCISION MADE REQUIRED SUTURES.
PULLING WRENCH TO TIGHTEN BOLTS ON BELT SCRAPER.
