Fabick CAT employee was working in the Cleveland Cliffs Mining Shop. The employee was working on a motor grader cylinder head, installing a fitting. The wrench slipped off and the employee's hand hit a flange, on the cylinder head, causing a small laceration on the top of EE's left hand.

Employee was removing valve from Tilden 1 #2 South deaerator line when their left hand was pinched between the valve and a handrail. The employee was wearing gloves at the time of incident and sustained a laceration to their left hand. Employee was transferred to local ED for evaluation and received medical treatment (7 sutures) and returned to work.

Employee was removing a fire water valve when the employee's left finger was pinched between the valve and the hand railing while lowering the valve with a chain hoist. Employee was transferred to local ED for imaging which revealed nondisplaced fracture to employee's left pinky finger. Employee was provided a splint for immobilization and protection and returned to work.

Employee was using a pry bar when working on a bus, removing the transmission. The bar slipped and struck EE in the mouth, causing the tooth filling to fall out and broke part of the tooth.

An employee experienced an incident while using a ratchet that became increasingly difficult to turn. During this process, the employee felt a pop in their right bicep. Following the incident, the employee was evaluated and was returned to work within their job class. However, the employee is now scheduled for surgical repair, which triggered reportability.

MMG's were removing the 5B rougher drum in the east side of the Concentrator to repair the tub. They used OH crane to remove drum to set the magnetic rougher drum on ball mill floor cover. The cover became stuck to bottom of drum. MMG jumped on 5ft pry bar placed between cover & drum (@ angle ~4-5" from ground), when cover released, EE injured right foot.

Employee was using brass punch to remove rod screen arm bearing. During this activity a piece of the punch broke off hitting employee's tooth. The employee did not visit a dental office until 9/10/25, and at which time, the tooth was repaired. The office visit summary wasn't received until 9/16/2025

While installing the #8 Support shaft, the safety holdback rigging was in place and not utilized. This allowed the support shaft to slip and pinch the employee's calf between the shaft and shaft support wheels. Communication between the crew and foreman was poor. Crew complacency decreased awareness

While chipping slag after a weld job, a railroad employee got debris in their eye, which had to be extracted at a local medical facility. Safety glasses were worn at the time of the incident. After the debris was extracted, EE was prescribed antibiotic eye drops as a preventive measure.

A welder, removing a lip shroud from a loader bucket, received a laceration to the rt thigh from flying debris resulting from steel-to-steel contact when striking the lip shroud with a sledgehammer. Employee received further evaluation and treatment in the ER.

Employee was attempting to adjust a pan chain of the stamler pan feeder. Two 1" nylon straps were attached to the pan and a come-along. Employee was ratcheting on come-along when the pan shifted causing the weight of the pan to rest on one of the nylon straps. This caused the nylon strap to break. The come-along struck employee in right rib area.

The Tech was performing a removal of a Y-PIN on a 993k Wheel Loader. They were pulling the Y-PIN with the PIN tool specified in the CAT SIS system. When pressure was applied the puller rod pulled the threads out of the end of the Y-PIN. When they moved the apparatus the bell slid off of the puller rod causing the weight of the rod to flip back and strike the tech in the face.

The injured employee was working on the #1C/M on the South Mains #2 Unit, changing the left side ripper motor in the #4 heading. The process of changing the motor requires a lot of physical movement and the use of small tools to aid in positioning. While prying to move the motor in place the injured employee suffered pain in right shoulder.

Plant repairman prying on a metal plate with a 4' pry bar. The pry bar slipped, causing tip of right ring finger to be pinched between the pry bar and side of metal plate. Employee sustained an approximate 1/2" cut to tip of finger. Seen by on-site EMT and transported to UP Health System - Bell Hospital, treated for laceration requiring 4 stiches.

The employee was using a steel wedge to remove the 5th row key on a concave. While using a sledge hammer to pound on the wedge a piece of metal shrapnel came off the edge of the key and lodged into the employee's right thigh. The shrapnel had to be removed by a physician and stitches were required to close the wound.

EE was attempting to remove a D7 undercarriage roller. EE was striking the flange of the roller with a 15lb. sledgehammer when a fragment of the roller flange broke off. The piece of metal struck EE in the left thigh resulting in a 1" laceration in the thigh.

EE trying to remove a section of cut bolt that was jammed between two feeder pans. EE had a drift punch in right hand and was striking it with a ball peen hammer in left hand. When the bolt dislodged, the pan on the east side jumped forward and pinched right thumb against the pan to the west. Taken to ER for stitches.

Mechanic was installing an idler cap on Cat dozer 69. Mechanic was using a ball-peen hammer to adjust the roller cap. While swinging the hammer it glanced off of track above the roller cap striking a finger resulting in a cut. Mechanic was seen by onsite EMT, and then brought to a hospital for further evaluation. Received stitches to laceration.

Utility knife blade cut through leather glove and skin of left hand middle finger. Employee was taken to a Hospital, stitches were required to close laceration to left hand middle finger.
