Employee was emptying a bucket near Tower 2; he was striking the side of a bucket with a sledge hammer to dislodge material. A small piece of rusted metal flew off the bucket striking the employee in the right lower leg causing a small laceration. The piece of metal had to be removed from the wound.
Injured employee was cutting a slot in a conveyor belt to be used as a side belt with a utility knife when the knife slipped and cut his wrist. The laceration required stitches. The injury did not result in lost time or restricted dury.
EE was assisting with clearing a blockage at Gryatory crusher and feed chute when a slab of rock slid down the chute striking right elbow. This resulted in an MSHA reportable accident because of one day restricted duty.
EE was removing wet material blocking an airlock using a 2.5' pry bar (wearing heavy gloves). While gouging out the material his left ring finger struck against a connection flange. This resulted in sutures which made it a reportable, no lost time accident.
Employee was placing a sign post into the ground. Due to under lying rock, when he hit post it moved. On the second hit, he hit his right index finger with hand-held sledge.
Employee was swinging a 25 pound sledge hammer to clear a dry ball from the seasoner when he felt pain in the center of his back and his right shoulder. Employee did not loose time until February 5, 2007 when the doctor diagnosed a torn rotator cuff.
Reparing water line with two shop EE's who were using a bar (~15lbs.)to raise line, the bar slipped and struck the EE on the right shoulder. The foreman was aware but EE did not think he needed medical attention. Monday when EE reported to work with pain in right should was sent to medical treatment. This resulted in MSHA reportable.
The injured employee was installing screw back into dust pump housing when he experience a strain while twisting and pushing. He was given a 15 pound lifting restriction by the doctor, which would not allow him to perform all of his duties as a maintenance mechanic.
Closing lid on pneumatic trailer using a 8' pole from catwalk above loadout chute. Leaned over the handrail to reach lid, it was stuck requiring extra force. He felt a slight pull in lower back but continued to work. Reported injury on 4/07/06 but did not seek medical attention until 4/11/06.
EE was tightening bolt with a rachet, the socket broke, EE fell backwards landing on his left side causing lumbar strain. This is a restricted duty report.
EE was performing maintenance on a horizontal roller conveyor using a hammer. The hammer slipped and hit his left index finger while he was holding a punch. No modified work available and his accident, will be a lost time.
While welding at jaw crusher, chipped slag from weld and slag flew in behind safety glasses into right eye.

Raking kilns when a large piece of coal ring fell off the discharge dam hitting the rake. The side of the rake struck him on the right side of nose resulting in stitches.
Employee was moving a 55 gal barrel of kerosene in the Process Bldg with a chain hoist. A clasp was hooked to the barrel edge and as it was moved from the pickup truck, the clasp released and the barrel dropped about two feet onto employees rt foot. Employee had a hairline fracture on top of foot; no medical treatment, just time off for swelling to go down.
Cleaning fuller multicones. bumped shovel causing lime to fall getting in eyes.
raking the fuller cooling gates when a piece of coal ring came off the fuller discharge dam hitting the rake, causing the rake handle to twist hit employee in the right ear.
EE was assisting with the removal for the fork attachment on skid steer. EE was using a 5' section of drill steel to break free/striking at the bucket release lever. EE missed the release with the bar overstressing his R shoulder/upper arm, resulting in a MSHA Reportable Restricted Duty Injury.
The employee was removing material buildup from the walls of the Pit hopper from a standing position above the hopper. He was using a hollow bar about 11' long and weighing about 25 lbs. He performed about 6 downward thrusts with the bar striking the hopper walls and when he continued the movement a 7th time, he felt pain in his lower back.
While chipping the welds on a steel fabrication, a metal particle must have gotten around the safety glasses and into his eye. Employee did not have any issues with his eye until he woke up the next morning. Injury was determined to be MSHA reportable because the doctor prescribed medicated eye drops.
The employee was attempting to support the transmission oil pan of a front-end loader with his hand. The mechanic broke the seal between the transmission oil pan and the transmission with a crowbar. The oil pan broke free and fell to the concrete slab. It pushed the employee's left hand down to the concrete slab and fractured the tip of the middle finger.
EE WAS USING A UTILITY KNIFE TO CUT CONVEYOR BELTING MATERIAL TO USE AS A LINER FOR A BELLY PAN. EE WAS PULLING THE KNIFE THROUGH THE MATERIAL WHEN IT SLIPPED OUT OF THE CUT AND WENT ACROSS HIS RIGHT UPPER LEG. EE RECEIVED STITCHES AND RETURNED TO WORK.
WHILE POUNDING OUT BOLTS ON TUBE COVER FOR SCN-50-102 AT DOLOMITE PRIMARY GRIZZLY, THE HAMMER SLIPPED OFF THE BAR AND STRUCK EE IN TOOTH, CHIPPING THE TOOTH.
EE was shoveling along 50W stacker area. While shoveling sand next to concrete wall, the corner of the shovel hit the concrete wall, stopping the EE's motion. EE's right hand went numb and his shoulder started hurting. Diagnosis from medical clinic is strained right shoulder.
EE WAS WELDING ON THE CHUTE AT THE FULLER KILN. SLAG WAS CHIPPED WITH A HAMMER AND A PIECE FLEW INTO HIS RIGHT EYE.
EE WAS USING UTILITY KNIFE TO CUT SMALL OPENING IN RUBBER CONVEYOR BELTING MATERIAL BEING INSTALLED AS AN END SKIRT BOARD ON A TYROCK WET SHAKER SCREEN. THE EE WAS IN THE PROCESS OF CUTTING A SMALL HOLE WHEN THE KNIFE SLIPPED OFF THE WORK AND STRUCK HIS LEFT INDEX FINGER, CUTTING A PORTION OF THE FINGERNAIL AND FINGERTIP.
EE WAS CUTTING OFF THE END OF A SILICONE SEALANT TUBE AND CUT HIS FINGER.
Employee was tapping on a bearing race with a hammer in order to remove it from the bearing. A sliver of metal from the bearing race broke off and embedded in employee's left thigh. Employee had to go to doctor to get sliver removed.
REMOVING A SEAT BELT FROM CAT LOADER, STANDING BENT AT WAIST INSIDE CAB USING RIGHT ARM FOR SUPPORT WHILE USING LEFT ARM TO LOOSEN BOLT. EE USING RATCHET WRENCH APPLYING TORQUE TO REMOVE BOLT WHEN WRENCH SLIPPED. THE WRENCH STRUCK THE EMPLOYEE IN THE LEFT EYE GOING UNDER SAFETY GLASSES.
THE DOCK LEADER STRAINED HIS BACK AS HE CLEANED THE FLUME WITH A HOE IN THE SOUTH DOCK FINAL SCREENING BUILDING.
THE INJURED WAS TAKING THE COVER PLATE OFF THE APRON SHAFT. AS HE DID THIS, THE WRENCH SLIPPED OFF THE BOLT HEAD AND HIS ELBOW STRUCK A STEEL COLUMN. HIS ELBOW WAS DRAINED ON 09/24/03, TO REMO VE THE SWELLING.
EE WAS WORKING TO REMOVE BLOCKAGE FROM A #1 SCREEN CHUTE AT THE MAIN CRUSHING. HE WAS USING A 12' CLEANING ROD THROUGH AN ACCESS PANEL. WHEN THEBLOCKED MATERIAL WAS DISLODGED, IT GAVE WAY AND PUSHED THE ROD THROUGH HIS HANDS AND INTO HIS STERNUM.
THE MILLWRIGHT WAS WELDING A CRACKED SKID PLATE ON A LOADER BUCKET. A RAM WAS USED TO PUSH THE STEEL INTO PLACE SO IT COULD BE WELDED. THE WELD ON THE STEEL STRAP THAT HELD THE RAM IN PLACE BR OKE CAUSING THE STRAP TO FLY AND STRIKE THE MILLWRIGHT IN THE FACE. HIS FOREHEAD WAS CUT AND NEEDED 6 SUTURES.
THE REPAIRMAN WAS SPLICING L-6 CONVEYER BELT WHEN HE STRUCK HIS LEFT THUMB WITH A HAMMER.
EE WAS MANUALLY TURNING GATE VALVE WITH A PIPE WRENCH AND WHILE PUTTING PRESSURE ON THE VALVE FELT PAIN IN LOWER BACK (LOWER BACK STRAIN).
2 EE'S WERE CUTTING A PIECE OF PIPE. WHILE ATTEMPTING TO HIT THE PIPE WITH A HAMMER, ONE EE INADVERTANTLY STRUCK THE OTHER EE'S LEFT HAND FRACTURING HIS THUMB.
EMPLOYEE WAS USING A 24 INCH PIPE WRENCH TO FREEA STUCK SHAFT. WRENCH SLIPPED THEN CAUGHT AGAIN. HIS HAND WAS BETWEEN THE PIPE WRENCH & HIS BODY & HE HEARD SOMETHING SNAP. HE WORKED THE NEXT 2 DAYS THEN DECIDED TO TELL HIS SUPERVISOR & WAS SENT FOR X-RAYS. IT WAS FOUND HE BROKE A BONE ABOVE HIS LEFT HAND LITTLE FINGER.
EE WAS WORKING TO REMOVE SUCTION HOSE FROM PUMP. AFTER UNSUCCESSFULLY TRYING TO SCREW IT OFF EE DECIDED TO CUT IT OFF. HOSE WAS MADE OF RIBBED PLASTIC. HE GRABBED LOWER PART OF HOSE WITH LEFT HAND & CUT LENGTHWISE WITH RIGHT HAND. POCKET KNIFE SLIPPED OFF HOSE & CUT LEFT THUMB CROSSWISE REQUIRING 3 STITCHES JUST BELOW KNUCKLE.
CRUSHER CHUTE WAS STOPPED UP. AFTER USING SLEDGE HAMMER TO UNSTOP IT, SHOULDER HURT WHEN HE MOVED IT.
EMPLOYEES WERE ATTEMPTING TO MOVE AN ID FAN MOTOR. ONE WAS HOLDING A 2' LONG 4X4 WOODEN POST WHILE KNEELING ON THE FLOOR. ANOTHER WAS USING A SLEDGE HAMMER TO ALIGN THE MOTOR WITH THE ID FAN. A 10'X4" HEAT SHIELD WEIGHING OVER 100 POUNDS FELL FROM THE IMPACT ONTO THE FIRST EMPLOYEE'S LEG.
PULLED MUSCLE IN LEFT LOWER SIDE WHILE TIGHTENING BRAKE ON RAILCAR. EE WAS PLACED ON RESTRICTED DUTY.
EMPLOYEE WAS ATTEMPTING TO CUT A LOCK OFF OF HIS LOCKER WITH A HACK SAW. THE SAW SLIPPED AND CUT HIS LEFT THUMB.
WHILE HAMMERING WITH 8 LB. SLEDGE HAMMER, HAMMER GLANCED OFF OBJECT AND STRUCK INSIDE EDGE OF RIGHT KNEE.
WAS TRYING TO SHUT RAIL CAR DOOR TO KEEP FROM LEAKING. USED SLEDGE HAMMER TO KNOCK TIGHT AND APPARENTLY SMASHED MIDDLE FINGER ON RIGHT HAND CAUSING POSSIBLE FRACTURE.
THE INJURED WAS CLEANING A KNIFE IN THE LUNCHROOM. AS HE WIPED THE KNIFE OFF WITH A PAPER TOWEL HE CUT HIS LEFT HAND MIDDLE FINGER.
THE INJURED STRUCK HIS LEFT SHOULDER ON A PIECE OF STEEL WHILE WORKING ON RC-9 CONVEYOR BELT. THIS INJURY BECAME LOST TIME ON 12/19/01.
STANDING ON CATWALK PUSHING UP ON A 14"LENGTH RATCHET. SHARP BURNING PAIN IN RIGHT SHOULDER AND NECK.
THE INJURED HAD CUT THE OLD BOLTS OFF THE IDLER BRACKET & WAS USING A HAMMER TO KNOCK THE BOLTS OUT. WHEN HE HIT THE BOLT THE WASHER FLEW UP ANDHIT HIM IN THE MOUTH WHICH CHIPPED HIS TOOTH.
