Employee was using a utility knife to cut a plastic bottle. The knife slipped and cut employee's right wrist - 3 stitches plus tetanus shot.

Employee was shoveling stone, the shovel hit something solid causing sprain to right wrist & forearm.

Employee was cleaning area around a feed plate using a steel bar. The bar slipped causing employee'e right hand to strike a dust suppressor fitting causing a fracture to his right ring finger.

He was helping mechanic and a metal splinter got into his right thumb.

Employee was holding a metal pipe under a rock hammer as it was being lowered into a box for shipping. As the hammer fell into box, the pipe was jerked downward causing back pain to employee.

An employee using a sledge hammer to tigten a wedge plate bolt struck fellow employee who was holding slugging wrench on left bicep when the hammer glanced off the wrench.

EE was using a sledge hammer while changing a shaker screen. His right index finger was crushed between shaker hopper and hammer when he missed the hit pin. He received a blackened nail.

EMPLOYEE WAS USING A PRY BAR BETWEEN DRUM AND CABLE WHEN BAR FLEW BACK INJURING HIS LEFT HAND.

EMPLOYEE WAS HOLDING A CRUSHER WEDGE DOWN WITH A PRY BAR WHEN HE WAS HIT IN THE ABDOMEN WITH A SLEDGE HAMMER BY A CO-WORKER WHO WAS HITTING THE WEDGE. THE HAMMER GLANCED OFF THE WEDGE & HIT HIM CAUSING A STOMACH CONTUSION.

EMPLOYEE WAS PRYING ON ROCKS IN CRUSHER WITH BAR AND THE BAR SLIPPED AND SMASHED RIGHT RING FINGER.

Ee was hit in mouth by a come-a-long handle that he was standing near, which shifted causing ee to cut his lip & chip his front teeth.

EMPLOYEE WAS KNOCKING PISTON OUT OF AN ENGINE WITH A HAMMER. THE PISTON CAME OUT AND PINCHED EMPLOYEE'S FINGER BETWEEN PISTON AND EDGE OF CRANKSHAFT CAUSING LACERATION REQUIRING 3 STITCHES.

EMPLOYEE WAS PERFORMING ADJUSTMENT ON STONE CRUSHER WITH A WRENCH WHICH WAS BEING STRUCK BY A CO-WORKER USING A HAMMER. EMPLOYEE'S FINGER GOT PINCHED BETWEEN WRENCH HANDLE AND CRUSHER FRAME.

EMPLOYEE WAS USING A UTILITY KNIFE TO CUT A PIECE OF RUBBER CONVEYOR BWLT WHEN HE CUT HIS LEFT INDEX FINGER TOP - 8 STITCHES.

EE WAS USING A SLEDGE HAMMER TO REMOVE A HYDRAULIC CYLINDER WHEN HE STRUCK HIS HAND WITH HAMMER.

EE WAS USING A CHISEL & HAMMER TO CUT A BROKEN BOLT. A PIECE OF THE BOLT HEAD BROKE OFF, STRIKING EE IN LT. FOREARM.

EE WAS CLEANING UNDER THE PRIMARY CRUSHER. WHEN HE THREW A SHOVEL FULL OF DIRT, HE FELT PAIN IN HIS LOWER BACK.

EE WAS POKING CLOGGED MATERIAL WITH A METAL BAR THAT WAS CAKED IN A CHUTE. THE BAR SLIPPED, SMASHING HIS FINGER BETWEEN THE BAR AND A HAND RAIL.

EMPLOYEE WAS MOVING A BULL SCREEN INTO POSITION USING A COME-ALONG. THE SCREEN PINCHED HIS RIGHT PINKY BETWEEN I-BEAM AND THE COME-ALONG HOOK.

WHILE LOWERING A CORE CHUTE ON TO A CONVEYER BELT, THE CHUTE SLIPPED PINCHING EMPLOYEE'S HAND BETWEEN CHUTE AND CONVEYER SIDE RAIL CAUSING SPRAIN AND BRUISE.

EMPLOYEE WAS HOLDING A LARGE WRENCH WHILE FELLOWWORKER STRUCK IT WITH A SLEDGE HAMMER TO LOOSEN A BOLT. THE FELLOW EMPLOYEE MISSED THE WRENCH AND HIT EMPLOYEE IN THE HIP AND ABDOMEN CAUSING CONTUSION AND BRUISES.

WHILE ADJUSTING A CRUSHER, EMPLOYEE PINCHED THUMB BETWEEN WRENCH AND SIDE OF CRUSHER.

WHILE TIGHTENING A 4 FT CRUSHER, EMPLOYEE STRUCK THUMB WITH SLEDGE HAMMER.

WHILE CLEANING STONE OFF CONVEYOR BELT, EMPLOYEE FELT TWINGE IN RIGHT HIP AREA.
