Employee was using a pry bar to lift heavy pieces of steal, the pry bar slipped out, causing over extension of shoulder.

While working from a ladder the ee was using a hammer to open the door at the bottom of a bin. As ee swung the hammer it caught an electric power cord which threw him off balance and he fell off the ladder.
