On 9-26-05 he requested medical treatment for same injury. Physician diagnosis: infected hematoma, employee not to work until further instruction from physician.
While splitting a piece of stone with a 4 lb. hammer and stone chisel, a metal fragment broke off the chisel puncturing employee's left wrist. Metal fragment was removed by a physician. Employee was placed on light duty for three workdays and returned to full duty on 9-20-05.

Diagnosis: traumatic conjunctivitis, prescription medication given. Returned to work next morning full capacity.

AS EE STRUCK NEAR THE EDGE OF THE SLAB, A PIECE OF STONE PROJECTED OFF THE SLAB STRIKING THE CALF OF LEFT LEG. INJURY WAS NOT REPORTED UNTIL 040105, MEDICAL DIAGNOSIS WAS CONTUSION TIBIAL AREA. LOST 1 WORK DAY 040105

Employee was splitting a slab of sandstone which was laying flat on the ground using a stone chisel and 6 lb. hammer. While attempting to strike his chisel the hammer first struck the ground causing him to strike his left thumb.

EE WAS USING A METAL PRY BAR TO LIFT AND POSITION A SLAB OF SANDSTONE FOR SPLITTING WHEN THE PRY BAR SLIPPED OUT FROM UNDER THE STONE. THE STONE FELL ON HIS LEFT FOOT. HE WAS WEARING FULL METATARSAL SAFETY SHOES. DOCTOR REPORTED NO BROKEN BONES BUT A CONTUSION OF THE LEFT FOOT.
Ee was splitting stone with a chisel and 6 lbs hammer. as he swung his hammer toward his chisel, the stone unexpectantly separated causing him to pinch his right index finger between the stone and his hammer handle.
