Miner was conducting maintenance work on front end loader bucket teeth. Miner was using a 3 lbs hammer to drive a pin when a metal fragment chipped off of tooth and became embedded into left bicep requiring medical attention.

Miner was injured by pry bar that was used as a guide for pulling a rope by a front end loader installing new feed belt on screen conveyor. Miner received contusion to left shoulder.

Employee was on top of primary feed hopper on the jaw crusher unclogging lodged material when the bar he was using missed its target and traveled downward pulling his hand with it causing it to get smashed between the bar and grizzly bar. This accident was not caused or contributed by neither any rule nor regulation not being followed, or any used PPE.
