Mechanic was removing ripper tooth from dozer with soft-faced hammer when a metal shard broke off and went through glove between right index & middle finger. 07/29/24 - Emp. developed infection in wound; medically treated and given antibiotic with return to full duty status.

Employee was using a digging bar to clean out material buildup inside the hopper underneath the screen at the midway plant. While doing so EE hit the top right of EE's hand on the metal drive gear box that stands out from the screen. Employee was taken for X-Rays and sustained a small fracture to EE's right metacarpal.

EE attempted to track a belt back under a wiper using a short metal pry bar while the belt was still running, the bar snagged on the belt and hit the employees right hand against a metal support lacerating his pinky finger requiring 8 sutures. All guards were in place at the time of the incident and the ee used a step ladder to elevate himself and reach over the belt guarding.

Miner was shoveling around conveyor support brackets when the shovel struck wooden blocks used to support the conveyor support legs. Impact with the wooden blocks caused injury to the miner's right wrist.

Employee was cutting a 1/2 inch air hose off of a reel. The knife he was gutting it with slipped and the emoplyee cut his right index finger.

Shop employee was working on a piece of equipment and struck himself in the mouth with a wrench, resulting in chipping a tooth. Minor crack in the tooth, no other injuries sustained.

Miner cutting hose with knife while holding hose in hand. Knife slippped and cut hand. Gloves were not worn while cuttting hose.

While waiting for another employee to finish a job, injured employee decided to remove a tail-pulley on a cross-conveyor. Employee tried to remove pulley with prybar, when left hand index finger was pinched between tail-pulley and frame.

Employee was changing the teeth on the bucket of the excavator. He was loosening a tooth with a sledge hammer. A small piece of metal broke off and struck the employee in the stomach.

Utility knife cut through a piece of conveyor belt easier than expected. Utility knife had new blade and employee applied too much pressure. Placed on modified duty.

EE was using porta power (Jack) to spread slat conveyor; block slipped and porta power jumped up and hit EE on the upper lip, cutting upper lip.

Employee was using a folding razor knife to cut wire ties. As he attempted to stand, he lost his balance causing him to strike himself in the left knee with the knife. He received 9 sutures to close the laceration.

EE was sweeping a concrete slab, when the metal handle on the broom folded. EE continued to use the broom when the damaged area on broom pinched his left pinky finger (tip) and caused a laceration that required seven stitches.

EE was removing sprocket from drag chain inside clarifier, support chain come-a-long internal mechanism failed causing chain to fall and pinch EE's Left hand between sprocket and drag chain.

Individual was cutting a piece of rubber backing with a razor knife. The knife went through the rubber and lacerated his left thumb.

While adjusting comealong EE over adjusted and had thumb in the wrong place. His left thumb was caught between comealong and angle iron.

EE WAS STRUCK IN THE FINGER WHILE USING A HAMMER RESULTING IN A LACERATED PINKY FINGER REQUIRING.

EE USING COME-ALONG WENCH TO PULL TWO SLEEVED PIPES APART. THE INNER 4" STEEL PIPE WAS RUSTED AND RESISTED SMOOTHLY SLIDING OUT OF LARGER PIPE. WHEN IT FINALLY RELEASED IT TRAVELLED OUT 32" AL LOWING THE STEEL PIPING AND CROSS-BRACING TO DROP ON EMPLOYEE'S LOWER LEG AND ANKLE.

THE MECH WAS WELDING A PLATE ON THE REAR OF A HAUL TRUCK. ANOTHER EE WAS TAPING THE PLATE INTO PLACE WHEN THE HAMMER SLIPPED STRIKING THE TOP OF THE MECHANIC'S HEAD. THE MECH FINISHED THE SH IFT WITH LITTLE COMPLAINT. THAT EVENING HE DECIDED TO SEEK MEDICAL ATTENTION. THE RESULT WAS A SLIGHT CONCUSSION. EE WAS OFF 8/6/02 AND RETURNED 8/7/02.

EE WAS PRYING STUCK MATERIAL FROM AJW WHEN THE BAR SLIPPED FORWARD CATCHING HIS FINGER BETWEEN THE PRYBAR AND A CROSS MEMBER OF THE JAW.

WHILE TAKING OFF BOLTS WITH RECHET/WRENCH ON NUT, WRENCH SLIPPED AND EMPLOYEE FELL BACK INTO FRAME OF EQUIPMENT.

EE WAS HAMMERING ON A BAR W/ DOUBLE JACK TO BREAK A PIECE OF OVERSIZE CONCRETE INT HE CONE CRUSHER BAR HEAD WAS MUSHROOMED AND 1/8" PIECE OF METAL PENETRATED HIS ARM.

AN EE WAS CUTTING RUBBER CONVEYOR BELTING WITH A UTILITY KNIFE. WHEN HE CUT THROUGHT THE END OF THE BELT THE KNIFE LACERATED HIS RIGHT KNEE, BELOW THE KNEE CAP.

AN EE HIT HIS RIGHT FOOT WITH A SLEDGE HAMMER WHILE CHANGING ANVILS IN A VERTICAL SHAFT IMPACTOR. THE HAMMER GLANCED OFF THE ANVIL AND STRUCK HIS FOOT.

EE USING 3/4" DRIVE RACHET WITH A 4 FOOT CHEATER PIPE TO ADJUST BELT TENSION NUT. APPARENTLY HIS GRIP SLIPPED ALLOWING END OF PIPE TO STRIKE HIS FOREHEAD.

EMPLOYEE USING A SLEDGE HAMMER TO STRIKE A WRENCH BEING USED TO TIGHTEN A LARGE NUT. EMPLOYEE MISSED WITH SLEDGE AND STRUCK HIS RIGHT FOOT.
