Mechanic was removing ripper tooth from dozer with soft-faced hammer when a metal shard broke off and went through glove between right index & middle finger. 07/29/24 - Emp. developed infection in wound; medically treated and given antibiotic with return to full duty status.

Employee was using a digging bar to clean out material buildup inside the hopper underneath the screen at the midway plant. While doing so EE hit the top right of EE's hand on the metal drive gear box that stands out from the screen. Employee was taken for X-Rays and sustained a small fracture to EE's right metacarpal.

EE attempted to track a belt back under a wiper using a short metal pry bar while the belt was still running, the bar snagged on the belt and hit the employees right hand against a metal support lacerating his pinky finger requiring 8 sutures. All guards were in place at the time of the incident and the ee used a step ladder to elevate himself and reach over the belt guarding.

Miner was shoveling around conveyor support brackets when the shovel struck wooden blocks used to support the conveyor support legs. Impact with the wooden blocks caused injury to the miner's right wrist.

Employee was cutting a 1/2 inch air hose off of a reel. The knife he was gutting it with slipped and the emoplyee cut his right index finger.

Shop employee was working on a piece of equipment and struck himself in the mouth with a wrench, resulting in chipping a tooth. Minor crack in the tooth, no other injuries sustained.

While waiting for another employee to finish a job, injured employee decided to remove a tail-pulley on a cross-conveyor. Employee tried to remove pulley with prybar, when left hand index finger was pinched between tail-pulley and frame.

Employee was changing the teeth on the bucket of the excavator. He was loosening a tooth with a sledge hammer. A small piece of metal broke off and struck the employee in the stomach.

Employee was using a folding razor knife to cut wire ties. As he attempted to stand, he lost his balance causing him to strike himself in the left knee with the knife. He received 9 sutures to close the laceration.

EE was sweeping a concrete slab, when the metal handle on the broom folded. EE continued to use the broom when the damaged area on broom pinched his left pinky finger (tip) and caused a laceration that required seven stitches.

EE USING COME-ALONG WENCH TO PULL TWO SLEEVED PIPES APART. THE INNER 4" STEEL PIPE WAS RUSTED AND RESISTED SMOOTHLY SLIDING OUT OF LARGER PIPE. WHEN IT FINALLY RELEASED IT TRAVELLED OUT 32" AL LOWING THE STEEL PIPING AND CROSS-BRACING TO DROP ON EMPLOYEE'S LOWER LEG AND ANKLE.

THE MECH WAS WELDING A PLATE ON THE REAR OF A HAUL TRUCK. ANOTHER EE WAS TAPING THE PLATE INTO PLACE WHEN THE HAMMER SLIPPED STRIKING THE TOP OF THE MECHANIC'S HEAD. THE MECH FINISHED THE SH IFT WITH LITTLE COMPLAINT. THAT EVENING HE DECIDED TO SEEK MEDICAL ATTENTION. THE RESULT WAS A SLIGHT CONCUSSION. EE WAS OFF 8/6/02 AND RETURNED 8/7/02.
