Struck by flying object

Mechanic was removing ripper tooth from dozer with soft-faced hammer when a metal shard broke off and went through glove between right index & middle finger. 07/29/24 - Emp. developed infection in wound; medically treated and given antibiotic with return to full duty status.

Heavy Equipment Mechanic was welding a small crack by the mounting bracket of the 980M wheel loader's fuel tank. A flash of fire occurred and mechanic received burns to face, chest, and left arm.

Struck against a moving object

Miner experienced lower back pain while excavating sample material holes. Excavator occasionally hit hard material which caused a jarring sensation when front end of excavator would lift off the ground & land back down once bucket became free. Miner requested to visit WC Doctor on 8/10/18 due to lower back pain not subsiding. Physician placed miner on light duty.

Employee sustained burn to right ankle when a piece of slag fell into a crease in jeans on top of boot trim; slag burned through the material and dropped down into the boot while performing welding repairs. Physician placed employee on 'off work' status.

Over-exertion in lifting objects

Mechanic was working out of a man lift and leaned against the hand rail while handing another employee a steel plate, the employee help a pop and sharp pain at his lower rib cage. The employee requested to see a doctor on 1-6 and was diagnosed with a sprained lower rib on his right side. He is on restricted duty with a recheck on 1-9-12.

Shop employee was working on a piece of equipment and struck himself in the mouth with a wrench, resulting in chipping a tooth. Minor crack in the tooth, no other injuries sustained.

Struck by rolling or sliding object

Employee was working under work truck to adjust transmission linkage when truck started rolling and hit employee on the right side of his head causing a laceration. Employee was released with no restrictions.

LOADER OPERATOR DROVE OVER BUMPS THAT CREATED A ROUGH AND VIOLENT FORCE AGAINST HIM CAUSING THE NECK STRAIN.
