Employee was using a digging bar to clean out material buildup inside the hopper underneath the screen at the midway plant. While doing so EE hit the top right of EE's hand on the metal drive gear box that stands out from the screen. Employee was taken for X-Rays and sustained a small fracture to EE's right metacarpal.
Employee was in the process of changing wear iron on a cone crusher. While installing the feed plate into the bottom of the crusher, and getting it into its final position, the metal plate slipped unexpectedly, pinching their right ring finger between the feed plate and the screen deck.
Employee was lifting a belt motor on to an in place belt line for future installation. As ee went to set it on the belt with the help of another employee left middle finger was pinched between the motor and the belt causing a laceration on the pad of finger.
EE was driving a water truck for dust abatement when suddenly the seat dropped to the floor causing him back pain.
EE was grinding with a portable grinder, when the wheel slipped off the work surface and his forearm contacted a sharp edge of the conveyor frame.
Person was cleaning a conveyor guard.
Employee stepped down to a platform from the screen area and twisted his left ankle.
Individual was cutting a piece of rubber backing with a razor knife. The knife went through the rubber and lacerated his left thumb.
EE WAS EXITING DOZER, SLIPPED ON TRACKS AND STRUCK THE DOZER TRACK WITH HIS UPPER TORSO AND BACK.
AN EE WAS CUTTING RUBBER CONVEYOR BELTING WITH A UTILITY KNIFE. WHEN HE CUT THROUGHT THE END OF THE BELT THE KNIFE LACERATED HIS RIGHT KNEE, BELOW THE KNEE CAP.
EMPLOYEE WAS CONNECTING A CHAIN TO A BOBCAT, WHEN THE OPERATOR OF THE BOBCAT BECAME INNATTENTIVEAND MOVED THE BOBCAT FORWARD RUNNING OVER THE EMPLOYEE'S LEFT ANKLE AND LOWER LEG.
