A 12KV power line failure resulted in a 4.9 acre brush fire. A communication cable was damaged.

Caught in, under or between a moving and a stationary object

While waiting for another employee to finish a job, injured employee decided to remove a tail-pulley on a cross-conveyor. Employee tried to remove pulley with prybar, when left hand index finger was pinched between tail-pulley and frame.

While walking out from under conveyor, ee came up too soon and hit hardhat on conveyor beam. This caused ee to lose conscieness and fall to ground. His neck and jaw were injured.

Employee was holding impact gun over his head to conduct maintenance on horizontal crusher. While doing this, he strained right shoulder.

EE was using gas welding torch to perform maintenance on conveyor, when a piece of hot slag entered around his wrap-around safety glasses and contacted his left eye.

WHILE OPERATING LOADER IN THE PIT AND FEEDING THE CRUSHER, EMPLOYEE SAID THAT THE SEAT ON THE LOADER WAS BROKEN. SUPERVISOR LOOKED AT THE SEAT AND FOUND SOME WEAR IN THE ADJUSTMENTS BUT THERE WERE NO LOOSE PIECES AND SEAT WAS NOT LOOSE.
