While working in the shop, EE dropped a wrench on left hand causing a closed non-displaced finger fracture. The adjustable wrench landed on EE's hand which was caught between the wrench and metal container.

EMPLOYEE WAS ADDING AN EXTENSION TO THE WATER TRUCK SPRAY BAR. HE HAD REMOVED THE SPRAY BAR & CLAMPED IT INTO A VISE & WAS TIGHTENING ON A NEW SECTION. THE PIPE SLIPPED OUT OF THE VISE & STRUCK EE IN THE HEAD. EE RECEIVED A CUT & SLIGHT CONCUSSION. LOST TIME.
EE was building a new conveyer. The ratchet he was using broke and ee fell into the conveyer frame, striking his right shoulder. The shoulder popped when he hit the conveyer. The Dr suspects a torn rotator cuff, is currently on restricted duty.
EE WAS SHOVELING AROUND A SHUT DOWN AND LOCKED OUT CONVEYER. HIS SHOVEL HIT SOMETHING AND JAMMED HIS SHOULDER. HE SAW AN ELECTRICAL CABLE AND ASSUMED THAT HE HAD BEEN SHOCKED. (RESTRICTED DUTY AND RX MEDS) AN ELECTRICIAN WAS BROUGHT IN TO CHECK IF THERE COULD BE ANY LIVE WIRES OR VOLTAGE TO THE FRAME AND THERE WAS NONE.
EMPLOYEE WAS HAMMERING A ROCK OUT OF THE FEEDER AND HIT HIS FOOT WITH A SLEDGE HAMMER. CONDITIONS REQUIRING REPORTING: RX PAIN KILLER/SPLINT/RESTRICTED DUTY.
EE WAS HAMMERING ON THE GATE LATCH OF THE ACCESS DOOR OF THE BELT FEEDER. THE LATCH BROKE AND STRUCK EE IN THE FACE. THIS CAUSED A LACERATION OF THE LEFT CHHEK AND ABOVE THE LEFT EYE. MEDICA L ATTENTION 13 STITCHES.
EE WAS BENDING A PIECE OF REBAR TO USE AS FILL FOR A WELD. HE STUCK THE BAR IN A HOLE IN THE WELD TRUCK AND WAS PULLING ON IT. THE BAR SLIPPED OUT OF THE HOLE AND HE WAS STRUCK BY THE BAR.
EE WAS SPLITTING A PIECE OF RUBBER HOSE WITH A UTILITY KNIFE. THE KNIFE SLIPPED OFF THE HOSE AND CUT HIS HAND.
