Employee was removing a pin connecting the portable crusher to the loader. When tapping out the pin with a hammer and pry bar, the tongue of the portable crusher moved towards the employee causing the pry bar to lacerate the employee's left index finger. The employee required stitches to treat the laceration.

Employee was changing blow bars on a HSI crusher. EE used a small sledge hammer to hit one of the blow bars causing a piece of the blow bar to chip. The piece hit EE just inside EE's right nostril. The employee was taken to the emergency department where the piece of metal was removed and EE was given one stitch.

The employee was removing an old shut off valve from the fuel truck while on one knee and two hands on the pipe wrench, his right hand pulled the wrench and he felt something on his arm/ shoulder joint that started to feel pain. This was being treated as a doctor visit injury until surgery was diagnosed to be scheduled 4/3/2013 he was working with restrictions.

The valve on the hopper was stuck in the closed position. Employee open the port hole to remove the rocks that were blocking the path and secured the port hole. He proceeded to strike the hopper with a mallet and the port hole flung open, the material sprayed out getting behind his safety glasses into both of his eyes.

EE was trying to close bottom hopper door on coal car, using a 20 ton hydraulic jack to close last few inches to pin in place. Bottle jack slipped, causing door to spring open and pinch EE finger between the door and grizzley grating.

EE and co-workers were removing the existing platform on a tire feed system. The platform was cut loose and held by a chain fall. EE was holding the hand rail or the platform when the platform moved pinching the ring finger on his right hand between the hand rail and a diagonal brace resulting in laceration and broken bone of his right ring finger at the first joint.

Plant lead man was patching an outer jacket on the trommel screen. He was using a bolt like a hammer to punch a bolt through a hole in the patch. He missed the bolt on this particular swing and struck his left hand injuring his ring and pinky fingers. He suffered lacerations to both fingers requiring 10 stitches and also dislodged his pinky nail.

Employee was installing a chute on the load out bin, utlizing a come a long. Come a long slipped & the chute twisted, which caused the ee's hand to be caught between the chute & the come along. Employees left index finger was severly cut and smashed. Injury resulted in stitches to the finger. Employee was wearing personal protective equipment, when the accident occured.

Miner was using a belt punch to put a hole in a piece of skirtboard rubber. Upon striking the 5-6 blow he struck punch w/ a glancing blow. Punch angled to side of blow punching his left index finger between punch & rubber that he was working on. Miner was wearing gloves, however the force of punching action resulted in tear of skin on his left index finger resulting in 10 stitches.
