Fall from machine

The injured employee jumped from a manbasket and received a cut on his head and lip. The employee was taken to the doctor and received first aid treatment (there were no stitches, butterfly bandages were applied to the wound). The doctor placed the employee on restricted duty.

Installing return roller (plant 7-grit belt)-employee struck roller with hammer to make it line up with bracket, little finger got pinched between roller and bracket, cutting finger.

HE HAD TO GO TO THE HOSPITAL OVERNIGHT BECAUSE HE WAS HOLDING WIRES WITH A PVC PIPE AND HE TOUCHED A LIVE ELECTRICAL WIRE.

HE WAS WORKING A PIPE; THEY PICKED UP ON THE PIPE A COUPLE OF TIMES. WHEN HE PICKED UP THE PIPE THE THIRD TIME, THE RUBBER CAME LOOSE AND THE RUBBER PUSHED THE PIPE, AND HIS HAND GOT CAUGHT B ETWEEN THE TWO.
