Was shoveling out mud out of the secondary crusher when the shovel hit the inside of the lip on the crusher causing the shovel to stop abruptly and bending their right wrist back causing immediate pain.

Employee was changing the tooth on an excavator bucket. He was beating the pin out of tooth with a hammer and chisel when a piece of metal splintered off and struck him on the inside elbow of his left arm causing a laceration and embedding a piece of metal in the soft tissue.
Hit right middle finger with pry bar. Hand was up against a piece of machinery. 2 cm laceration to the top of right middle finger on MIP joint. 0.5 avulsion of skin on posterior MIP joint, no repair needed to this injury.
While attempting to release tensionon a chain boomer. The boomer released hitting EE in the lower leg breaking his leg.
EMPLOYEE WAS PRYING ON A BOULDER WHILE STANDING ON A LADDER. THE PRY BAR CAME LOOSE AND HE LOST HIS FOOTING AND FELL APPROXIMATELY 4 FEET LANDING ON A ROCK.
HOLDING A PUNCH TO DRIVE OUT A PIN.
EE WAS SERVICING A HAUL TRUCK. HE ATTEMPTED TO GREASE FITTING. FITTING WOULD NOT TAKE GREASE SO HE TRIED TO EXERT PRESSURE ON GREASE GUN. AFTER SERVICING THE TRUCK HE FELT PAIN IN HIS LEFT SHO ULDER. PER DR. INSTRUCTIONS, LIMIT LIFTING TO 30LBS.
