EE was chiseling a piece of stone and a chip flew. A piece of the chisel got lodged into his neck. The dr removed the metal sliver from his neck.

He hit his left hand index finger with a hammer, while splitting a stone.

EE DID NOT REPORT ACCIDENT RIGHT AWAY. HE TOLD OTHER ON FRIDAY 6-27-03 THAT A PIECE OF METAL FLEW FROM HIS CHISEL AND HIT HIS CHEST/STOMACH AREA.

WAS KNOCKING DOWN STONE & HE TWISTED WRONG.

HE WAS CHISELING ON A LEANING SLAB OF STONE AND WAS STANDING IN FRONT OF IT (INSTEAD OF THE SIDE OF IT) WHEN IT SLID DOWN AND PINNED HIS RIGHT FOOT UNDER THE STONE AND BETWEEN THE GROUND. HE B ROKE HIS FOOT.

WAS WORKING OUT ON THE BLOCK WHEN HIS CHISEL SLIPPED AND HE HIT HIS RIGHT THUMB WITH HIS HAMMER.

WAS CHISELING A STONE AND A PIECE GOT UNDERNEATH HIS SAFETY GLASSES AND IN HIS RIGHT EYE.

WAS CHISELING A STONE, WHEN A PIECE OF THE CHISEL FLEW UP AND HIT HIM ABOVE HIS RIGHT EYEBROW. FOREHEAD.

SAID HE WAS WORKING WITH STONE & WHILE SPLITTINGONE HE FELT HIS BACK PULL IN UPPER RIGHT.

LEFT HAND STRUCK WITH QUARRY HAMMER SEVERE SWELLING ACROSS THE TOP OF HAND BROKE LEFT 2ND METACARPAL.
