Miner was using a punch and hammer to remove a bolt in a screen cloth. A piece of metal went through the miner's glove and imbedded in left hand.

A rock wedged in feeder, employee was using a sledge hammer to break the rock. The hammerhead came off handle mid swing and their right hand ring finger slammed down and hit the rock and steel causing fracture to finger.

Employee was tightening bolts on loader cutting edge. The edge slipped catching employee's watch wrist band causing lacerations to left hand.

Employee was holding slug wrench while tightening crusher bolts. Another employee hit wrench. IE lost control of IE's grip on wrench and smashed left hand between wrench and crusher.

Employee was using pry bar to loosen screens. Pry bar slipped & hit on the head. Cut required 6 staples.

Employee using 5 ft pry bar to position crusher toggle inside primary jaw crusher. Toggle shifted into place unexpectedly. Left index finger was pinched against crusher frame causing laceration and non-displaced fracture.

Re-installing window into rubber moulding, knife slipped and cut right index finger.

While using pry bar to free rock from clogged chute, rock released hitting left thumb causing contusion and fracture.

Holding punch with left hand--missed punch and hit left index finger with hammer causing laceration to left index finger.

Removing rivets from I-beam with a sledge hammer & a punch. Employee was hit on his right thumb by the sledge hammer.

EMPLOYEE WAS REMOVING CUTTING EDGE. WHEN HE CUT THE BOLT & REACHED FOR HAMMER, CUTTING EDGE FELL ON HIS FOOT.

COMEALONG SLIPPED, SMASHING FINGER AGQAINST DRILL MAST.

HELPING TO INSTALL BEARING USING HAMMER, MISSED PUNCH STRIKING FINGER BREAKING & CUTTING FINGER.
