Fall from machine

Exiting cab of dump truck, foot slipped off of step causing to lose balance and fall. While trying to catch themself hit concrete floor with hand/arm resulting in fracture of wrist.

Employee was assisting in repair of conveyor idler. Another employee tipped idler from horizontal to vertical position. It over traveled on to employees right hand and smashed hand.

Strained right knee climbing into back of pickup truck on 3-13-17. Had swelling and discomfort, saw doctor, recommended physical therapy with no lost work time. Condition persisted resulting in decision to have surgery on 6-7-17.

Employee using 5 ft pry bar to position crusher toggle inside primary jaw crusher. Toggle shifted into place unexpectedly. Left index finger was pinched against crusher frame causing laceration and non-displaced fracture.

While waiting for truck to load from overhead bin, employee was standing behind truck when a small rock fell and bounced off tailgate, hit front tooth breaking tooth.
